2639527 Primary Facility 2017-09-25 Plugging(Includes Plugged/Mined Through) No Violations Noted Arrived onsite at 09:35 am. Meeting at 10 am with PADEP and CNX.

Meeting held at well head. Meeting attended by Doug Catalano, April Weiland, James Braunns, Paul Wyrwas. From. CNX, Patrick Taylor, Mich Goodwin, Bill Dechere, Spence Johnson, Steve Spitzer. 13 inch pipe is below the concrete cellar We are looking at a 11 inch to 13 inch cap. CNX is going to cut the vent monument off and perform a video camera, temparature and bond log test of the open 9 5/8 inch pipe to see if the gas is coming from behind the 9 5/8 inch or from the cement plug located from 795 ft to 937 feet or bottom plug of the 9 5/8 inch pipe. CNX is going to revise its drawing of the well head. CNX will submit a schedule to you of field activities and give daily reports of findings Readings were 0 ppm HS and 1 ppm CO and 20.5 percent oxygen in breathing zone. At the welll head where the two inch vent was removed, a reading of 5 % Gas, 100 % LE, 19.5 % Oxygen, 0 ppm HS, 46 ppm CO was recorded.

2639744 Primary Facility 2017-09-25 Plugging(Includes Plugged/Mined Through) No Violations Noted Top of cement was 530 feet from grade. Conserv used 58 sacks of class A to bring cement to surface.
2731976 Primary Facility 2018-05-21 Plugging(Includes Plugged/Mined Through) Pending Well plugged in September of 2017. Cynthia Wihram onsite and noted poor growth of vegetation around former well and bubbling in pond area near well. Dep personnel Paul Wyrwas used an Altar 5 and recorded no readings near the bubbling or near the former well head Oxygen levels read 20. 6 percent. Water and mud present around former well head. High level of deer activity near former well head. Standing water pond was noted at the site. Called Josh Williams from EQT and requested him to inspect site and explain the Poor vegetation growth around well and and bubbling. Booms are still present.
2732619 Primary Facility 2018-05-21 Routine/Complete Inspection Violation(s) Noted At time of inspection well is plugged. Compost Filter Sock installed around site. According to Department records operator has until 6-25-18 to permanently restore site. It appears site has been seeded and mulched however large areas of the site remain un-vegetated (photo #1 and 2). These areas contain locations where deer activity has created holes several inches deep (photo #3). Soil probe readings taken in 4 of these holes indicate elevated soil conductivity levels.

In addition, Department observed bubbling in mud on site (photo #4). This bubbling is constant and continues even after mud is disturbed. DEP OGI Paul Wyrwas did not record any readings with an Altair 5X meter at the location of the bubbling. No odors were observed. Department contacted operator rep Josh Williams by phone from site. Mr. Williams stated he will visit site today to assess the situation. Operator rep Vanessa Crane contacted Department by phone on 5-21-18: left message. Department returned call on 5-22-18. Ms. Crane stated operator intends to visit site on 5-23-18. Ms. Crane stated she would follow up with Department on 5-23-18 or 5-24-18 regarding site visit findings. Violations issued on 5-21-18 remain outstanding. A follow up inspection will be conducted at a later date.

2734751 Primary Facility 2018-05-24 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2732619 where violations were issued for for failure to collect brine or other fluids and failure to dispose of residual waste.

Operator rep Vanessa Crane contacted Department by phone. Mr. Dave Allison was also present on call. Ms. Crane summarized operator site visit of 5-23-18. Ms. Crane stated plugged well buried plate was unable to be located. Gas was bubbling from site, no gas readings were recorded with their meter. Gas samples were obtained and sent for analysis. Soil probe readings were taken throughout the site. Some readings were elevated (3 locations); others were not. Soil samples were not collected by operator. Ms. Crane stated there is a 2" gathering line near site. This line will be surveyed next week. Ms. Crane stated a map of soil probe locations recorded by operator will be emailed to Department next week. Mr. Allison stated the buried plate will be located, exposed, and gas readings taken/samples collected. Department recommends operator follow u

2735233 Primary Facility 2018-05-29 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection to inspection record #2732619 where violations were issued for for failure to collect brine or other fluids and failure to dispose of residual waste.

At time of inspection location of buried plate is marked with a wooden stake. Large areas of site remain un-vegetated (photo #1). No bubbling observed at this time. Department collected soil samples; 4 in area of potential impact and 2 background. Operator rep Josh Williams on site during inspection. Mr. Williams stated the buried plate will be exposed and inspected on Thursday (5-31-18). Violations issued on 5-21-18 remain outstanding. A follow up inspection will be conducted at a later date.

Department received and reviewed soil sample analytical results (attached). Based on this review, it appears a release of production fluids has occurred. Department recommends operator determine the cause of the release, determine the nature of the released fluids, and ascertain if release is still occurring. Department requests operator update Department as information is obtained. Violations issued on 5-21-18 remain outstanding. A follow up inspection will be conducted at a later date.

At time of inspection areas of site remains un-vegetated (photo #1). Location of buried plate surrounded by security fence (photo #2). Plastic pipe appears to be venting the plugged well. This pipe contains water and intermittent bubbles are observed coming to the surface at this time. Violations issued on 5-21-18 remain outstanding. 2778570 Primary Facility 2018-09-21 Follow-up Inspection Outstanding Violations - Viols Req'd FUI to inspection record #2732619.

At time of inspection it does not appear that any site remediation or plugging preparation activity has taken place (photo #1). Plugged well surrounded by security fence. At this time gas is escaping through the fluid in the plastic pipe that appears to be venting the well (photo #2). No bubbling is observed in the muddy, un-vegetated areas on site. Violations issued on 5-21-18 remain outstanding. A follow up inspection will be conducted at a later date.

2834020 Primary Facility 2019-01-31 Plugging(Includes Plugged/Mined Through) Violation(s) Noted This well was plugged and is presently leaking gas to the atmosphere. A plan of action to drill out and re-plug is presently being undertaken and the well is to be re-plugged shortly. On the date of this inspection, EQT field staff were present at the well and were in the process of monitoring the well for a flow test.

A violation of statute 3220(a) and Ch. 78.91(a) has been issued on this well for failure to plug the well in a manner to stop the vertical flow of gas within the well bore.

An additional violation of Ch. 78a.75 has been issued for failure to follow the approved Alternate Method calling for the well to be plugged solid with cement from the attainable TD to surface. The operator did use gel spacers between the cement plugs in an interval that was determined not to have produced gas or oil which was not a part of the approved Alternate Method.
