2491013 Primary Facility 2016-06-01 Routine/Complete Inspection No Violations Noted Vegetative cover on site consists of perennial vegetation with a density greater than 70%. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion and sedimentation was observed.

A 2-year extension of time to restore the site was granted by the Department on February 22, 2016. This extension extends the deadline to restore the site under Title 58 Pa. C.S. § 3216 (2012 Oil and Gas Act)(relating to site restoration) until February 22, 2018. A condition of the restoration extension requires that post-construction stormwater management (PCSM) BMPs be installed by May 22, 2016, unless additional time is requested, and approved, in writing by the Department. At the time of inspection, no PCSM BMPs were in place, and no evidence such as staking or flagging were present that would indicate the imminent construction of such BMPs. Further, the Departmen't s records do not indicate that a request to extend the deadline to implement PCSM BMPs was received or approved.

2502457 Primary Facility 2016-07-15 Routine/Partial Inspection No Violations Noted Partial inspection to assess status of post-construction stowmater management (PCSM) BMPs required as part of the extension of time for site restoration approved by the Department on February 22, 2016.

The PCSM BMPs have been installed as approved. Temporary Stabilization and erosion and sediment control BMPs are in place.

No violations were cited during this inspection.

2590222 Primary Facility 2017-05-04 Routine/Complete Inspection No Violations Noted Two Wells in production phase. Two each sand and water separators. No leaks detected from production equipment. Site stable. No violations noted.
2741905 Primary Facility 2018-06-15 Routine/Complete Inspection No Violations Noted Two wells staged for production phase. Two each sand and water separators. No leaks detected from production equipment. No violations noted.
2843434 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:37; the weather was ~25°F, sunny, and breezy.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with ice. There were also 2 GPUs, sand separators, and produced water tanks onsite. There appeared to be a fresh water injection tank set up to inject into the wells.

There were 3 infiltration areas constructed around the site. Areas closer to the pad were sufficiently vegetated and appeared stable. Areas farther from the pad surface were still snow covered. There was some sparse vegetation beginning to grow over the pad surface.

No violations noted at the time of inspection.
