2446105 Primary Facility 2016-02-02 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.

Temporary erosion and sediment control BMPs are in place around the pad, but have been removed along the access road. Overall, the site has >70% vegetative cover, though some patches with less cover exist. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2542616 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. No Chesapeake representatives were present.

Temporary erosion and sediment control BMPs are in place around the pad, but have been removed along the access road. Overall, the site has greater than 70% vegetative cover, with some areas that have less. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2561749 Primary Facility 2017-02-14 Routine/Complete Inspection No Violations Noted Arrived @ 11:50. Weather: snow and ice on ground, sunny. No personnel were on site. There were two wells in production on a 6-slot set-up.

Roundtop 2H: water and ice in cellar, no bubbling. Top gauge: 1028.4 psi (digital) 13-3/8” x 9-5/8”; venting through 2” check valve, shut to dump line, no methane detected at valve openings. 9-5/8” x 5-1/2”: 54 psi (digital) 5-1/2”: 1125 (digital). There were no violations at the time of my inspection. Left @ 12:10.

2604091 Primary Facility 2017-06-14 Routine/Complete Inspection No Violations Noted Two wells in production phase. Two each sand and water separators. No leaks from production equipment. Site has e&s concerns with water retention and temp control removal. No violations taken at this time.
2675359 Primary Facility 2017-12-27 Routine/Complete Inspection No Violations Noted Two Wells in production phase. Two each sand and water separators. No leaks detected from production equipment. No violations noted.
2729660 Primary Facility 2018-05-14 Routine/Complete Inspection No Violations Noted Arrived at 1349. Weather: sunny and nice. There are two producing wells with lift assist on a 6 Slot set up. All cellars are full of water.
2805505 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:35; the weather was ~40°F, calm, and the ground was snow covered. A work area was plowed around the production equipment.

There were 2 properly tagged Marcellus wells staged for production. There was a total of 6 well cellars onsite and filled with water; no bubbling was observed. There were 2 each GPUs, produced water tanks, and sand separators onsite and in secondary containment. The containment around the produced water tanks contained a small amount of frozen water.

The snow cover prevented the proper assessment of vegetative cover. However, large amounts of taller grasses and vegetation were visible above the snow. No obvious E&S issues were observed.

According to Department records the ESCGP for this site expired on 7/25/15. There does not appear to be any active well permits or restoration extensions for this site. It is recommended that an NOT be submitted in the spring or summer if this site is ready for permit termination.

No violations noted.
