Recently constructed pipelines run along the access road and off the north side of the well site. These areas still have temporary E&S BMPs in place.

The Department's records indicate that no current well permits exist for the site and no extension request for site restoration has been submitted or approved. In accordance with the information you provided to the Department by letter dated September 21, 2015, this site has been identified as a site for which a restoration extension request would be submitted. This information was submitted in response to the Department's June 26, 2015 letter requesting your plans to comply with Section 3216 of the 2012 Oil and Gas Act and 25 Pa. Code Section 78.65, which relate to site restoration. You are reminded that such an extension request should be submitted as soon as possible.

No voilations were cited during this inspection.

2542291 Primary Facility 2016-12-06 Routine/Complete Inspection No Violations Noted Remarks Arrived @ 11:20. Weather: cold and cloudy. There were no personnel on site. There were 3 wells capable of production on the pad. The Joan Clark 2H was in production. The 1H and 5H wells were not in production at the time of my inspection. For all of the wells the 13-3/8” x 9-5/8” annuli were plumbed to the dump line. The 9-5/8” x 5-1/2” annuli were shut to a digital gauge, all of the gauge readings were < 0 psi. There were a few feet of water in the Joan Clark 1H cellar. The other two cellars were full. There was no bubbling in any of the cellars. No violations were observed at this time. Left @ 12:00.

2776431 Primary Facility 2018-09-17 Routine/Complete Inspection No Violations Noted Chesapeake Appalachia submitted a Notice of Termination (NOT) to the Department for the Joanclark Pad covered under ESX 10-113-0017. The NOT was received by the Department on 9/12/18.

There are 3 Marcellus wells staged for production & properly tagged (1H, 2H, 5H). There are also 2 produced water tanks, 3 GPUs, & 3 sand separators onsite & in secondary containment. At the time of inspection there was significant rust/staining on the produced water tanks (Photos #5, #6) and the top of the GPUs (Photo #7).

There is 1 PCSM infiltration area along the northern edge of the pad. There is a grass/gravel two track road between the pad and the infiltration area. At the time of inspection the infiltration area appeared to be installed as depicted on PCSM/SR plans.

There were minor amounts of erosion on the access road (Photo #2). The rest of the site was well vegetated and no other E&S issues were observed.
