Temporary erosion and sediment control BMPs have been removed. Overall, the site has >70% vegetative cover. No evidence of accelerated erosion or sedimentation was observed. No staining or other evidence of spills was observed on the pad surface. No puddles were present on the pad at the time of inspection, so electrical conductance was not screened.

This well site is due for restoration. However, Chesapeake indicated in its filing dated September 21, 2015, that a restoration extension request would be submitted for this site. At this time, the Department's records do not include an extension request. This request should be filed as soon as possible.

Temporary erosion and sediment control BMPs have been removed. Vegetative cover on the site is greater than 70%. No evidence of accelerated erosion or sedimentation was observed. No staining or other evidence of spills was observed on the pad surface. No puddles were present on the pad at the time of inspection, so electrical conductance was not screened.

A request for an extention of time for site restoration was filed with the Department on April 22, 2016. To date, the submitted request has not been approved.

2643938 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted Arrived at 12:15: Weather: sunny, very nice. There was 1 producing well on a 3-slot set-up. All of the cellars were filled with water.

Rain 5H 13-3/8" x 9-5/8": open to dump line, 2.83 psi (digital) 9-5/8" x 5-1/2": closed to dump, -0.26 psi (digital 5-1/2": 356.85 psi (digital) Top gauge: 1282 psi ( digital). Left at 12:50. No violations were observed at the time of my inspection.

2805500 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:45; the weather was ~34°F, calm, and the area was snow covered.

The access road and an area around the wells was plowed. There were several puddles in the plowed area. Conductivity of these puddles was below 100 µS/cm. The snow cover prevented assessment of site stabilization but there was a considerable amount of taller grasses and vegetation visible above the snow.

There was 1 properly tagged Marcellus well staged for production. There was a total of 3 well cellars onsite and filled with water; no bubbling was observed. There was 1 GPU, 1 sand separator, and 2 produced water tanks onsite and in secondary containment. There was some water in the produced water tanks containment that was frozen.

According to Department records the ESCGP for this site expired on 10/7/15. It is recommended that once site stabilization has been achieved and any erosional issues resolved an NOT be submitted in the spring or summer.
