No evidence of accelerated erosion or sedimentation was observed. The electrical conductance of puddles on the well pad was not screened due to the absence of puddles.

Currently, no active well permits have been acquired or are pending. The Erosion and Sediment Control General Permit 2 for the site expired on 03/17/2015. In order to remain in compliance with the Department's regulations regarding site restoration (Title 58 Pa. C.S. § 3216) an active well permit should be acquired or a 2-year extension for site restoration should be requested.

No violations were noted during this inspection.

The PCSM BMPs have been installed as approved by the Department in February 2016 through the granting of a 2-year extension of time for site restoration. Erosion and sediment control BMPs and temporary stabilization have been installed, and no evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

There is 1 properly tagged Marcellus well staged for production. There is 1 GPU onsite and in secondary containment. There are 2 produced water tanks in secondary containment. There was moderate corrosion noted around the bottoms of the tanks (Photo #3).

Both PCSM BMPs were constructed as depicted on the PCSM plans. At the time of inspection there was little to no water in the infiltration basins. The site was >70% vegetative cover. No E&S issues were observed.

It does not appear that an NOT has been submitted for this site. It is recommended that if the site meets restoration criteria and the site is ready for permit termination an NOT be submitted to the Department.
