2362779 Primary Facility 2015-04-16 Routine/Complete Inspection No Violations Noted : Arrived @ 8:30 and met OGI Ken Kennedy on location. This was a three slot set up with one well in production. The well in production was the Quail N Sul 3H. At the start of the inspection a Chesapeake rep was flushing out this well with fresh water which went to the production tanks. A new blank well sign was in place but not filled out, the Department recommends this be addressed . There were 4 annular vents on the Quail N Sul 3H. 20” x 13-3/8” : vented with a check valve; no gas detected. 13-3/8” x 9-5/8”: -1.34 psi on digital Scada gauge. 9-5/8” x 5-1/2” ,plumbed to production tank : 2.4246 psi on Scada gauge, plumbed to production tank, 5-1/2”,1800 psi, not plumbed in. A portion of the well pad in front of the Quail N Sul 3H, was excavated. The excavation was related to a3/24/15 methanol release (10-12 gallon methanol/water estimated release) that was reported by Chesapeake (CTS # 310203, see printout).
2422893 Primary Facility 2015-11-04 Routine/Complete Inspection No Violations Noted Arrived @ 13:33. No activity on site. 1 well in production, 2 other cellars, one filled with gravel. This is the Quail S Sul 2H, a conductor-only spud that was recently plugged.
2843426 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:17; the weather was ~25°F, sunny, and breezy.

There was 1 properly tagged Marcellus well staged for production and 1 plugged well. The well cellar was filled with ice. There were 2 produced water tanks, 1 GPU, and 1 sand separator onsite.

There were 3 infiltration areas installed. The exposed areas showed sufficient vegetation however, much of the site was snow covered. Some sparse vegetation was beginning to grow onto the pad surface. The side of infiltration area #3 showed signs of erosion (Photo #5). There was also a section of filter sock in place near infiltration area #3.

The Department recommends stabilizing and repairing any areas of erosion and removing unneeded temporary E&S BMPs. It is also recommended that an NOT and any needed restoration reports be submitted if and when appropriate.
