Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. A request for a 2-year extension of time for site restoration was filed with the Department of Environmental Portection On July, 25, 2016. To date, that request has not been processed.

No violations were cited during this inspection.

The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 7/2/2018. Based on this inspection report the Post Drilling Restoration report was accepted on 7/5/2018.

2843449 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:27; the weather was ~20°F, calm, and sunny. There was about 1 inch of snow covering most of the area. The gate was open when I arrived.

There were 3 properly tagged Marcellus wells staged for production. There were also 3 GPUs, 3 sand separators, 2 produced water tanks, and 2 compressors onsite. The compressors were not running during the inspection. An extra tank vault containing methanol was also onsite and did not appear to be hooked up.

Much of the site was snow covered however, the containment berm was bare and appeared to be well vegetated. One infiltration berm was observed during the inspection. The only obvious E&S concern was noted by the access road entrance at the culvert outlet (Photo #4).

The Department recommends stabilizing and repairing any areas of erosion as needed.
