2478763 Primary Facility 2016-04-27 Routine/Complete Inspection No Violations Noted Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. The electrical conductance of puddles on the well pad was not screened due to the absence of puddles.

Currently, no active well permits have been acquired or are pending. The Erosion and Sediment Control General Permit 2 for the site expires on 05/13/2017. In order to remain in compliance with the Department's regulations regarding site restoration (Title 58 Pa. C.S. § 3216) an active well permit should be acquired or a 2-year extension for site restoration should be requested.

No violations were noted during this inspection.

2542596 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. No Chesapeake representatives were present.

Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The cellars at the well heads were observed to be completely full of water. The water should be removed to prevent overflow.
