There are 4 cellars on the pad with two wells drilled but not fracced: SE 1H, 113-20174 & NE 1H, 113-20175. Pressures for the SE 1H were: 5 ½ tubing: 0 psi; 9 5/8 x 5 ½ : 0 psi; 133/8 x 9 5/8 : open & venting. Pressures for the NE 1H were: 5 ½ tubing: 0 psi; 9 5/8 x 5 ½ : 0 psi; 133/8 x 9 5/8 : open & venting. The inspection was concluded at 10:01.On 1/20/15 at 14:49, OGIs Rick Swank and Becky Renner returned to the location following a 75-hour shut-in period. Pressures for the SE 1H were: 5 ½ tubing: 0 psi; 9 5/8 x 5 ½ : 0 psi; 133/8 x 9 5/8 : 0 psi. For the 133/8 x 9 5/8 the concentration of combustible gas was 175 ppm. For the 9 5/8 x 5 ½, 400 ppm combustible gas was measured. Pressures for the NE 1H were: 5 ½ tubing: 0 psi; 9 5/8 x 5 ½ : 0 psi; and 133/8 x 9 5/8 : 0 psi. 15:20, inspection concluded.

One side of the fenced-in well area was open. There were 4 cellars on the pad with two wells, drilled but not fracced: SE 1H, 113-20174 & NE 1H, 113-20175. For the SE 1H, wellhead valves were chained and secured, water in the cellar was frozen above the level of the conductor. Pressures for the SE 1H were: 5 ½ production casing: 0 psi; 9 5/8 x 5½ : 0 psi; 133/8 x 9 5/8 : open & venting, no methane was detected. For the NE 1H, wellhead valves were chained and secured, water in the cellar was frozen above the level of the conductor. Pressures for the NE 1H were: 5 ½ production casing: 0 psi; 9 5/8 x 5 ½ : 0 psi; 133/8 x 9 5/8 : open & venting, no methane gas was detected. The inspection concluded at 13:15.

The Department's records indicate that a Restoration Extension Request was submitted on 09/11/2015. This information was submitted in response to the Department's June 26, 2015 letter requesting your plans to comply with Section 3216 of the 2012 Oil and Gas Act and 25 Pa. Code Section 78.65, which relate to site restoration

2710533 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I met with Kevin Rothlisberger when I arrived. Mercer Well Services workover rig was on the site, preparing to drill out plugs on the NE Sul 1H well; plugs already drilled on the SE Sul 1H. Rig mats covering the liner made it impossible to assess, but the berms appeared to be in good condition. Water puddled on containment near the freshwater tank and a Baker tank battery showed elevated conductance (greater than 19.99 mS/cm); the volume was difficult to determine, as the puddle ran under the rig mats, but was at least 25-50 gallons. A smaller amount of water was visible at the liner over the earthen berm at the back of the pad; conductivity ranged from 6.9-7.1 mS/cm. The volume was even harder to determine, as a strip only 2-5" was visible before it ran under the rig mats. Overall housekeeping was otherwise good. I mentioned the areas of elevated conductance to Kevin, and he said he would have them vacced as soon as possible.

There were 2 properly tagged Marcellus wells staged for production. A total of 4 cellars were onsite and filled with ice. There were 2 GPUs and sand separators onsite. There were also 2 produced water tanks onsite and in secondary containment.

The pad remained at its full size. The berm along the western edge of the pad contained a small amount of snow/ice (Photo #1) and the basin along the access road was mostly full with water (Photo #4). Some patches of snow remained but large portions of the area were visible and appeared to be well vegetated. No E&S issues were noted.

No violations observed at the time of inspection.
