Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2491015 Primary Facility 2016-06-01 Routine/Complete Inspection No Violations Noted Vegetative cover on site consists of perennial vegetation with a density greater than 70%. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion and sedimentation was observed.

A 2-year extension of time to restore the site was granted by the Department on February 22, 2016. This extension extends the deadline to restore the site under Title 58 Pa. C.S. § 3216 (2012 Oil and Gas Act)(relating to site restoration) until February 22, 2018. A condition of the restoration extension requires that post-construction stormwater management (PCSM) BMPs be installed by May 22, 2016, unless additional time is requested, and approved, in writing by the Department. At the time of inspection, no PCSM BMPs were in place, and no evidence such as staking or flagging were present that would indicate the imminent construction of such BMPs. Further, the Departmen't s records do not indicate that a request to extend the deadline to implement PCSM BMPs was received or approved.
