2753458 Primary Facility 2018-07-18 Routine/Complete Inspection No Violations Noted Weather was 70 and sunny at time of inspection. Met with Chesapeake representative Joe Foerste on location at time of inspection. Rig was in the process of drilling the 12 1/4" hole for the intermediate casing. The surface casing was set on 7/16/18. Reviewed the cement job log for the surface casing and no discrepancies were noted. They were intending on cementing intermediate casing at approximately 12:00 on 7/19/2018. Conducted a walk around inspection of location. No violations were noted at time of inspection.
2758792 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted Weather was 71 and cloudy at time of inspection. Met with Chesapeake representative Neil Brown on location. This well was completed on 7/30/18. Reviewed the cement job log for the production casing. No discrepancies were noted at time of inspection. Well was shut in with a night cap and covered at time of inspection. No violations were noted at time of inspection.
2768392 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted Weather was 75° and sunny at time of inspection. There were no Chesapeake representatives on location at time of inspection. Well was closed in at time of inspection. Well was not yet stimulated. Well had a 0-10k analog pressure gauge attached to the top of the wellhead with a needle valve below it. The gauge read 0 psi at time of inspection. There were no violations noted at time of inspection.
2790920 Primary Facility 2018-10-17 Routine/Complete Inspection Pending I arrived onsite and was unable to locate a Chesapeake company representative. I spoke briefly with one of the workers on the site, who informed me that the frac had ended that morning, and that equipment removal was beginning. Puddles located on the pad surface outside of containment were generally low in conductivity (.6-1.1 mS/cm). Between the freshwater AST and the upright brine tanks in containment, however, I identified an area where high conductance water (19.42 mS/cm) was seeping slowly from under the containment liner. The affected area was approximately 2'x5' and very muddy with little free water, but I was able to collect a sample (sample ID# 0939076) by scraping out a small depression where the water was flowing and bailing the fluid into the sample containers with the container caps. I strongly recommend that Chesapeake attempt to determine the source of this fluid, as the volume of fluid present under the liner is unknown. The result of the inspection will be left as Pending until sample
2804050 Primary Facility 2018-11-16 Administrative/File Review No Violations Noted Sample results received from sample ID# 0939076, taken on 10/17/18. Results show elevated barium, strontium, and chlorides, indicative of a release of wastewater to the pad. The Department would like to encourage Chesapeake to report all spills and releases within 2 hours of discovery, regardless of volume. The result of the Administrative inspection is Violation Noted.
