1815065 Primary Facility 2009-07-22 Follow-up Inspection No Violations Noted On July 21, a call was received from Chesapeake with a report of fluid seeping out of the bank of the reserve pit. Chesapeake collected samples of the fluid which was reported to visibly be clear and odorless, and was likely groundwater seeping below the pit liner. As a precaution, the fluid was pumped out of the pit to determine if there was a tear in the pit liner.
1819287 Primary Facility 2009-08-07 Follow-up Inspection No Violations Noted At time of inspection, I met with Company Man Bill Canon. The drilling rig on site is Nomac Rig 112. Mr. Canon indicated that drilling depth was at 5997¿ which is the approximate depth of the horizontal kickoff point. Drilling was being performed using air. Mr. Canon provided the charts of the BOP tests. Based on a review of the charts, it appeared that the appropriate tests were performed and the BOP is in working condition. During an inspection of the well pad, I observed some drilling mud on the pad surface near the base of the substructure next to the water tank. There was also an excessive amount of drilling mud on the pad between the substructure and the shaker units. I notified Mr. Canon of the areas of concern, and requested the mud to be cleaned up, and to implement some measures to keep the site as clean as possible and prevent further spills of this nature.

2712138 Primary Facility 2018-03-29 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection on the Benscoter well pad on March 29, 2018. The well pad was stable as was the access road. The cut and fill slopes as well as the infiltration basin were all well vegetated. The wells were properly and clearly labeled. The wells associated with this site are as follows: 115-20068, 115-20182.

There were no erosional issues apparent.
