2717608 Primary Facility 2018-04-12 Routine/Complete Inspection No Violations Noted I inspected the Nikolyn well pad on 4/12/2018. I performed a RTNC inspection. The wells associated with this site are as follows: 115-20296, 115-20297, 115-21015, 115-22439, 115-22440, 115-22441, 115-22461. The cut and fill slopes were well vegetated. The cut slope had a repair protected with a mulch blanket. There have been previous repairs to the cut slope repaired with #4 stone. The access road and pad surface were stable and solid. The wells were properly labeled. There were no erosional issues noted.

There were no violations observed.

2741638 Primary Facility 2018-06-13 Routine/Complete Inspection No Violations Noted Arrived on location at 11:00. United Drilling had (2) spudder rigs onsite setting 20" conductors for three new wells to be drilled on pad, the Nickolyn 1H, 3H & 6HC. An existing, previously plugged 20" conductor has had steel cap and gravel removed in preparation to re-enter this well slot under a New well permit, the Nickolyn 7HC (115-22460).

- United Drilling has completed drilling (1) new conductor to a TD of 100' and plans to be drilling, setting and cementing all (3) new 20" conductors to ~100'. All new conductor slots were drilled on air. - There are (2) existing wells on this pad which were previously in production, the Nickolyn 2H & 5H. Both these wells have been temporarily shut-in and disconnected from all Production equipment in preparation for drilling new wells on pad. Both these wells had protective cages installed over production trees. Both wells had (3) 2" annular vents present. The (5/P) vents were shut-in to gauges reading: 2H: 20 psi.; 5H: 80 psi. 2773716 Primary Facility 2018-08-23 Drilling/Alteration No Violations Noted I arrived at the site at 13:15. Patterson Drilling Rig # 580 is on the location. The weather conditions are partly sunny skies and breezy with temperatures around 70 degrees Fahrenheit. I met with Danny Lambert, site company man, consultant of CHK and David Newman, Drilling Superintendent of CHK. Mr. Lambert and Mr. Newman provided me with the following information regarding site activities. "The current drilling depth on the Nickolyn 3H well is (651.97) feet. We just finished cementing the surface casing at (652) feet last night. We have waited on cement to cure for (8) hours and picked up the bottom hole assembly. We used a Halliburton Class H cement. A total of (29) barrels of cement returned to surface. The surface string was drilled with municipal water. The intermediate string will be drilled to (2,600) feet. We will drill with air/mist to (6,791) feet, which is at the top of the curve. We will load the hole with about (400-500) barrels of oil based mud. The well will be drilled to total d
