2467644 Primary Facility 2016-03-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35 p.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 68psi on it and the valve going to the tank was closed. There was 256psi on the production casing and the well tender well blowing the well into a tank to get the fluid out of the well so there was no reading on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:15 p.m.

2549633 Primary Facility 2017-01-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 15:00p.m., the cellar was full of water with no bubbling noticed. The (13x9) vent pipe was open and venting to the production tank, the (9x5) vent pipe was open with 79psi on it and the valve going to the tank was closed. There was 256psi on the production casing and 773psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 15:55p.m.

2633375 Primary Facility 2017-09-07 Site Restoration No Violations Noted I inspected the Hilltop site for Notice of Termination on August 8, 2017. There was well activity on site with Deep Well Services at the time of inspection. according to an employee of Deep Well Services, they were pulling the tubing from the wells and replacing it with smaller tubing. The site was solid and stable otherwise. The access road was solid and all vegetation was at or exceeding 70% cover. All vegetation was perennial. Instrument filing was complete and responsible parties were noted on the NOT form.

There were no erosional issues observed.

There were no violations noted.

I recommend APPROVAL of this NOT.

2637614 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2724695 Primary Facility 2018-04-30 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Hilltop well site. The wells associated with this site are as follows: 115-20407, 115-20408, 115-20409, 115-20410, 115-20411, 115-20412. The wells were properly labeled.

The cut and fill slopes were very well vegetated. The well surface was solid and stable, as was the access road. There were no erosional issues observed.

There were no violations.
