2717617 Primary Facility 2018-04-12 Routine/Complete Inspection No Violations Noted I inspected the Milochik well pad on 4/12/2018 for a RTNC inspection. The pad surface and short access road were ponded but stable. The berm surrounding the pad was recently cut and re-mulched to allow pad water to enter the infiltration basin. The small cut slope was well vegetated. The wells were properly labeled. The wells associated with this site were as follows: 115-20433, 115-20440, 115-20555. There were no apparent erosional issues. There were no violations noted.
2829173 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted I arrived onsite at 1409 to conduct a site restoration inspection in response to an NOT submittal received from the Operator on 13NOV2018. The weather was cold and sunny, with about 2 inches of snow on the ground. The pad surface was plowed but very icy in spots. There was a multitude of equipment stored on the pad (sand trucks, pump trucks, spill response trailer, etc.), presumably for the well stimulation activities taking place nearby.

After performing an administrative review of the plan and a site inspection, I was unable to determine if site conditions meet the requirements for the termination of this permit due to substantial snow cover on most of the site's vegetated areas. Another inspection will have to be performed when conditions permit to determine whether this submittal should be acknowledged or returned.
