2462867 Primary Facility 2016-03-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40 p.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 111psi on it and the valve going to the tank was closed. There was 28psi on the production casing and 945psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:35 p.m.

2692317 Primary Facility 2018-01-31 Routine/Complete Inspection No Violations Noted I inspected the Decker well pad on January 31, 2017. The access road and pad surface were solid and stable as was the fill slope. The fill slope and area all around the well pad were solid and stable. The wells were all properly labeled and the area around the pad was engaged in active agriculture (hayfield).
