2449146 Primary Facility 2016-02-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:35 a.m., the cellar was full of ice, the (13x9) vent pipe was open and venting to a tank, the (9x5½) vent pipe was open with 4psi on it and the valve going to the tank was closed. There was 1038psi on the production casing and 830psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 10:05 a.m.

2492469 Primary Facility 2016-06-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35 a.m. for a post 72hr PBUT, met with lease operators Jeff Iliff and Joel Mincer. They were setting up the wells for a 72hr PBUT, all the annuli’s were shut in on the 5H including the production line at 13:00pm and set up with a digital recorder, the production line was shut in on the 2H at 13:00pm and the other annuli’s will be shut in and set up with a digital recorder after the cellars are pumped out. The cellars are being pumped out with a one inch pump threw the vent line going to the tank.

I left the site at 13:00 p.m.

2493915 Primary Facility 2016-06-16 Follow-up Inspection Violation(s) Noted At time of inspection, arrived on site at 15:10p.m. to conclude the 72hr. Pressure Build-up Test (PBUT) being performed on the 2H and the 5H as part of an ongoing migration investigation. The cellar had water at the bottom with no bubbling noticed. I was accompanied by Eric Rooney (DEP Geologist), we met with Joel Mincer (lease operator for Chesapeake) and Tom Messing of Leidos. The 5H was shut in on 6/13/16 at 13:00p.m. and the 2H was shut in on 6/13/16 at 16:00p.m. Eric used an Altair 5X gas meter calibrated on 6/15/16 and Tom used a Gas Explorer gas meter calibrated on 6/16/16 to check for gas show at all annuli’s. Gas samples were taken if gas was detected. Pressures were taken from the SCADA digital pressure gauges on the wells.

I left the site at 16:30p.m. VIOLS for 25Pa Code § 78.86 Defective Cement at the (13x9) annuli.

2655044 Primary Facility 2017-11-01 Site Restoration No Violations Noted I inspected the Delhagen wellsite for notice of termination (NOT) on October 30, 2017. The access road was solid and well groomed. The cut slope had growth on it but not heavy vegetation, due to the rocky nature of the soil. The fill slope was well vegetated, and met the 70% vegetative standard for perennial growth. There was heavy rainfall the previous day, and the pad surface had quite a considerable amount of standing water. The rough area calculation was approximately 100,000 sq ft for the pad. There appeared to be no erosional issues at the time of inspection and the area was solid and stable.

There were no violations noted.

I recommend APPROVAL of this NOT.

2716830 Primary Facility 2018-04-09 Routine/Complete Inspection Pending Arrived onsite at 09:15. Onsite to witness the start of 72 hr. pressure build up test "PBUT" being performed on the Delhagen 2H & 5H wells on pad. Upon arrival I met with Chesapeake "CHK" Lease Operators, John Prevost and Evan Bresee. Upon inspection of wells, both wells (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") annuli have been open and venting. The 2H well had a 4" liner installed. This wells (4"x 5 1/2") annulus has been open and venting. In an effort to duplicate the last 72 hr. PBUT performed on these wells, this (4"x 5 1/2") annulus will remain open and venting through this PBUT. Both wells (Prod. X Tubing) annuli and Tubing prod. pressures were being monitored by SCADA pressure monitors. Both wells will remain in production through this PBUT period.

- Prior to shutting in annuli Tubing "TP", Prod. casing "PC" and Prod. Line "PL" pressures were recorded. See individual well pressures listed below. CHK Reps. performed flow checks using an electronic hand held manometer through a 0.25" orifice fitting. No

2794055 Primary Facility 2018-10-11 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived onsite at 09:05. Weather: ~ 55 degs., Raining. Onsite for conclusion of 72 hr. pressure build up test "PBUT" being performed on the Delhagen 2H & 5H wells on pad. Upon arrival I met with Chesapeake "CHK" Lease Operator, Evan Bresee. I was accompanied by DEP Geologist, Eric Rooney and Leidos Reps. Tom Messing and Shawna Erway. Both wells (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") annuli and the 2H wells (4"x 5 1/2") annulus were shut in on 10/8/18 @ ~ 09:30. Both wells (Prod. X Tubing) annuli, Tubing prod. pressures and shut in annuli pressures were being monitored and recorded by SCADA pressure monitors. Both wells remained in production through this PBUT period.

At ~ 09:30, Individual well annuli pressures were recorded, gas screenings were conducted, and samples were collected. Results are listed below: - 2H: (13x9) = 0.0 psi.; 30% LEL, 2% CH4. Leidos collected sample

          (9x5) = 1.5 psi.; 100% LEL, 35% CH4. Dept and Leidos collected sample.
          (5x4) = 0.3 psi.; No gas check performed.
