Gauges on 5 1/2, 9 x5" and 13 x 9" annuli. Gauge on 9 x 13" reads @ 25 psi. Other gauges show 0 psi. Cellar is dry. There is a PVC stand pupe extending from cellar floor to ground surface with cap. Wells are surrounded by concrete pad on surface. Violation cited for 78.86 - failure to prevent gas flow in annulus of Intermediate Casing.

Request Operator to identify source of gas venting to poly tank.

The site was well vegetated and stable. There was some small scale landowner removal of firewood near the site, but no disturbance to note.

2446306 Primary Facility 2016-02-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00 a.m., the cellar was about three quarters full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 13psi on it and the valve going to the tank was closed. There was 5psi on the production casing and 1259psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 11:30 a.m.

2594560 Primary Facility 2017-05-17 Routine/Complete Inspection No Violations Noted I inspected the Craige well site on May 16, 2017. The weather for the past few days had been quite rainy. There were puddles on the well pad and evidence of some small displacement of gravel on the access road to the site. No evidence of sediment leaving the site was observed however. All the slopes around the well pad were stable and were well vegetated with perennial grasses. There was an area on the fill slope which was too rocky to support plant growth but area was stable. The well cellars had no bubbling in them, and the site was stable overall. It would be recommended to touch up the road a bit.

2769423 Primary Facility 2018-08-30 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection of the Craige well site. The access road to the well pad is shared with a compressor station and has recently been regraded. The access road was solid and stable.

There is a pad below the well pad where various production equipment is stored. It was stable.

The wells on site were properly labeled.

The slopes were solid and stable with no erosional issues apparent and the vegetation was all lush and healthy. The soil was rocky in nature however the vegetation was growing profusely.

There were no violations noted.

2799381 Primary Facility 2018-11-06 Site Restoration No Violations Noted I performed a RTNC inspection of the Craige wellsite. The wellsite is located on a shared access road with a compresssor station and storage yard. This inspection concerns only the well pad.
    The weather was approximately 62 degrees F. and raining. 
    The access road was solid and stable, with no erosional issues apparent.  
    The pad surface was puddled and wet, owing to the conditions of rain.  It was solid, however.  
    The temporary BMPs have been all removed and all recorded PCSMs were present and well constructed.
    The slopes surrounding the well pad were all well vegetated despite rocky local conditions, with perennial vegetation exceeding the Department standard of 70% perennial vegetative cover, except in areas where there was insufficient soil to permit growth.
    The site was stable overall at the time of inspection, with no apparent erosional issues.
    There were no violations noted.
