1974985 Primary Facility 2011-05-18 Drilling/Alteration No Violations Noted Nomac Rig 245. Drilling surface pipe on fluid. Will besetting pipe at 714'. Cement prog.= 35 bbls gel, 100 bbls freshwater, 651 sks Class A w/ 2.5% CaCl/ 0.25lbs flake. pump rate to be 3-4 bbls/min. Discussed size of containment near solids mixing area. Should be increased. Drillers log on site and up to date.
1987213 Primary Facility 2011-07-06 Drilling/Alteration No Violations Noted Nomac 245 @ 600 ' above KOP. Mudded up alst night. was previously on air. Proposed TD = 13362'. Performed FIT @ 722' to 15 ppg and held for 30 mins. BOP test and FIT ok'd 6/24/11. Drillers log on site and up to date. Checked cellars. Milochik 3H has some bubbling coming to surgace. Operator will investigate and have cellar pumped down and notify DEP upon completion. Further investigation will be performed.
1989165 Primary Facility 2011-07-19 Drilling/Alteration No Violations Noted Nomac 245. At TD = 13 367'. Running 5 1/2'. Cementing prog shows cement will be brought up to @ 3589'. Installed Halliburton swellable packer @ 3544' and @ 4000'. had gas shows at 4200'.
2246879 Primary Facility 2014-02-20 Routine/Partial Inspection No Violations Noted Drilling Permit expired on 5/10/2012.
2717614 Primary Facility 2018-04-12 Routine/Complete Inspection No Violations Noted I inspected the Milochik well pad on 4/12/2018 for a RTNC inspection. The pad surface and short access road were ponded but stable. The berm surrounding the pad was recently cut and re-mulched to allow pad water to enter the infiltration basin. The small cut slope was well vegetated. The wells were properly labeled. The wells associated with this site were as follows: 115-20433, 115-20440, 115-20555. There were no apparent erosional issues. There were no violations noted.
2829178 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted I arrived onsite at 1409 to conduct a site restoration inspection in response to an NOT submittal received from the Operator on 13NOV2018. The weather was cold and sunny, with about 2 inches of snow on the ground. The pad surface was plowed but very icy in spots. There was a multitude of equipment stored on the pad (sand trucks, pump trucks, spill response trailer, etc.), presumably for the well stimulation activities taking place nearby.

After performing an administrative review of the plan and a site inspection, I was unable to determine if site conditions meet the requirements for the termination of this permit due to substantial snow cover on most of the site's vegetated areas. Another inspection will have to be performed when conditions permit to determine whether this submittal should be acknowledged or returned.
