2451338 Primary Facility 2016-02-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:10 p.m., the cellar was full of ice, the (13x9) vent pipe was open with 0psi on it and the valve going to the tank was closed and it was also set up with a pop valve, the (9x5) vent pipe was open with 0psi on it and the valve going to the tank was closed. There was 11psi on the production casing and 933psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:50 p.m.

2712146 Primary Facility 2018-03-29 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection on the Baltzley well site on 3/29/2018. The wells associated with this site are as follows: 115-20495, 115-20573, 115-20587, 115-20588 and 115-20591.

The wells were all properly and clearly labeled. The access road and well pad were wet but stable. There was some puddling on the pad surface due to recent rains. The vegetation surrounding the site was all thick and lush. The infiltration basin was empty. The topsoil piles and fill slope were stable with no apparent erosional issues.

There were no violations noted.

2769437 Primary Facility 2018-08-30 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Baltzley wellsite.

The access road was sold and stable, and the wellpad was stable and had no puddling.

All wells were properly labeled

The roadsides were well vegetated.

The slopes surrounding the site were all well vegetated, in excess of the 70 % perennial standard required. All the vegetation was healthy and lush.

There were no areas of instability noted on any of the slopes.

The infiltration basin PCSM was well constructed and stable with no apparent issues following the recent heavy rains.

There were no violations noted.
