2444383 Primary Facility 2016-01-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:25 a.m., the cellar was full of ice, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open with 0psi on it and the valve going to the tank was closed. There was 66psi on the production casing and 1197psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 10:40 a.m.

2588258 Primary Facility 2017-04-25 Incident- Response to Accident or Event No Violations Noted The Department arrived at the site at 11:35 in response to a notification of a deficient tank inspection at the site. The weather conditions were cloudy skies, with a slight to moderate breeze and temperatures around 50 degrees. There was no activity or personnel at the site. The notification was submitted to the Department on 4/24/17 at 10:28 PM. The notification indicates the following information. The date and time of the inspection was 4/23/17 at 8 AM. The tank contents is (400) bbls (barrels) of produced water. The reason/type of deficiency observed is a small hole was developed 4' from the top of the tank due to corrosion. The estimated volume lost of produced water from the tank is (31.5) gallons. The impacts resulting from deficiency were no impact, all fluid contained in secondary tank containment. The comments listed are all fluid was removed from secondary containment to be hauled for proper disposal/treatment. High level tank warning alarms were set at 250bbls for the tank and the well will ESD (
2592095 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted I inspected the Cook well site on 5/10/17. The site had recently been inspected by my colleague, Jon Sassi due to a deficient tank which had corroded causing a release to containment. The tank was still in place, and there was staining on the both produced fluid tanks indicating corrosional issues were ongoing. The containment did not have much fluid in it. The site was stable, with no evidence of any erosion problems occurring at the present time.

2607223 Primary Facility 2017-06-26 Routine/Complete Inspection No Violations Noted I inspected the Cook well site on 5/10/17. The site had recently been inspected by my colleague, Jon Sassi due to a deficient tank which had corroded causing a release to containment. The tank was still in place, and there was staining on both produced fluid tanks indicating corrosional issues were ongoing. Chesepeake has since indicated the tanks are all on a schedule to be replaced. The containment did not have much rainwater in it during time of inspection. The site was stable, with no evidence of any erosion problems occurring at the present time.

2754785 Primary Facility 2018-07-23 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Cook well pad. The well in production is the 115-20784. The plugged well onsite is the 115-20786. The active well was properly labeled.

The well site had a new produced water tank installed. There previously had been a steel tank which was patched extensively. The new tank appears to be fiberglass or plastic.

The access road was solid and stable with no erosional issues apparent.

The cut and fill slopes were vegetated where it was possible to grow plant life but they were mostly rock. They were stable and solid with no evidence of slope failure. The vegetation onsite looked healthy.

There were no erosional issues noted.

There were no violations noted.
