2621275 Primary Facility 2017-07-31 Complaint Inspection Pending The complainant reported activity at the site was impacting their quality of life by way of noise, vehicular traffic, and dust. Hydraulic fracturing operations had commenced at the site on or about 25JUL2017. Upon arrival I met with Oze Pruitt, the on-site Company Man, to notify and discuss the complaint I was investigating. Mr. Pruitt acknowledged that he understood my concerns and observations. He explained the silos were filled after each stage of the frack and were currently on stage ten of forty-nine. Mr. Pruitt informed me that he would coordinate with his immediate supervisor to address the issue as promptly as possible. I will conduct a follow-up inspection.
2624915 Primary Facility 2017-08-10 Complaint Inspection Pending I conducted the inspection accompanied by Jon Sassi, EPS. Upon arrival, we met with Mr. Deismond Bufford, Company Man on site. Mr. Bufford informed us the Hydraulic Fracturing (Frac) operations were currently between stages and 39 of 49 stages have been completed. Mr. Bufford informed us the expected conclusion and departure of the fracing operation should occur on or about 13AUG2017. A completions crew would arrive on site shortly after departure of the fracing equipment. I explained to Mr. Bufford I was conducting an ongoing complaint investigation concerning dust being generated by activity related to the operations. 2639169 Primary Facility 2017-09-22 Complaint Inspection No Violations Noted Upon arrival I met with Farrell Deslatte, on-site consultant representing Chesapeake Energy. Mr. Deslatte explained he was overseeing the installation of the flow lines to the GPUs (Gas Production Unit) and sand separators from the well. The installation was being conducted by Wood Group. Mr. Deslatte expected the work will completed by 27SEP2017. At the conclusion of my inspection I departed from the site. - Invalid Complaint Id. - 328527
2714745 Primary Facility 2018-04-05 Site Restoration No Violations Noted I inspected the Maris well site on April 5, 2017. The access road was very solid, and the pad surface was solid as well. The cut and fill slopes were solid and rocky. The wells were labeled correctly. Vegetation was prominent wherever it was possible for it to grow. There were no temporary BMPs left on site. There were no erosional issues noted. There were no observed violations. The wells associated with this site are as follows:

2782155 Primary Facility 2018-10-01 Routine/Complete Inspection No Violations Noted The well access road was firm and solid with no erosional issues. While on site I was approached by the landowner who seemed satisfied with the construction and maintenance of the well pad. The pad surface had some puddling from recent rains but was otherwise solid. The slopes around the well site were stable, but most of the area was simply too rocky to support growth. Where the vegetation was able to grow, it was perennial and exceeded the Department 70% standard. The PCSMs were all in place and the temporary BMPs surrounding the site were all removed. The wells were properly labeled.

There were no violations noted.
