2712687 Primary Facility 2018-03-30 Routine/Complete Inspection No Violations Noted I inspected the Borek well site on March 30, 2018. The access road was solid and stable. The pad surface was somewhat muddy due to recent rains. The wells were all properly labeled and the wells associated with this site were as follows:

115-21026 115-22426 115-22427 115-22428. The cut and fill slopes were very rocky but had vegetation where it was able to grow and there were no erosional issues observed.

There were no violations noted.

2758659 Primary Facility 2018-08-01 Drilling/Alteration No Violations Noted Upon arrival to location, (3) new 20" conductors have been set and cemented to surface for new wells, the Borek 2H, 4H and 104H. (1) open well cellar and (1) Existing well, the Borek S Sus 3H well has been shut in and disconnected from all production equipment. Liner and drilling mats have been installed in preparation for drilling new wells on pad.

- The Borek S Sus 3H well still had production tree installed. Pressure gauge installed on upper wing valve read 0 psi. at time of insp. (3) annular vents were present. The (5 1/2"/P) vent was shut in to gauge reading 0 psi. The (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") vents were open and venting to atmosphere through Sentry vent check fittings. Gas checks performed at these vents showed 0% LEL, 0% CH4. Well cellar had ~ 6" standing water, no bubbling was noted at time of inspection. See individual insp. for the Borek S Sus 3H for further info. - During this inspection no obvious concerns or violations were noted.

2774247 Primary Facility 2018-08-28 Drilling/Alteration No Violations Noted I arrived at the site at 11:26. The weather conditions are partly cloudy skies, a slight breeze, very warm and humid with temperatures in low to mid 80's. Patterson Drilling Rig # 572 is at the site. I met with Mickey Brewer, site company man, consultant of CHK, and Lauren Sorkin, Field Engineer of CHK. The Borek 4H well is currently in the process of being drilled at a depth of (838) feet. Mr. Brewer and Ms. Sorkin provided me with the following information pertaining to site activities. "We completed cementing of the surface casing at 11AM today, 8/28/18. This well will be drilled to a total depth of (12,710) feet. We are waiting on cement to cure for (8) hours. Schlumberger was the cement crew. We will resume drilling of the intermediate section tonight, 8/28/18. The Borek 2H was the most recent well drilled to total depth. The Borek 104H well will be the next well drilled to total depth. The Borek Sus 3H well was in production but is currently shut in." Mr. Brewer and Ms. Sorkin informed they wo
