2722341 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Susan well location. The well pad was solid as was the access road. The cut slope was stable and well vegetated. There was evidence of repair on the cut slope but the old repair was stable. The fill slope was solid and well vegetated. The wells were all properly labeled. The wells associated with this site are as follows:

115-20705 115-22446 115-22447.

There were no erosional issues noted.

There were no violations.

2754339 Primary Facility 2018-06-11 Drilling/Alteration No Violations Noted Arrived on location at 10:00. Onsite in response to new spud notices received for the Susan 4H & 5H wells. Upon inspection, (2) new 20" conductors have been set and cemented to surface. (1) existing well, the Susan 2H had previously been producing but has been disconnected from production and temporarily shut-in with a protective cage placed over this well in preparation for drilling of new wells on pad. (See Individual insp. for the 2H well for further details).

- Liner, Drilling mats and some containment has been set on pad. (7) Baker tanks have been staged on location in secondary containment in anticipation of future drilling of new wells on pad. - Overall, wells and location were found in good condition at time of inspection. Left site at 11:30.

2783964 Primary Facility 2018-10-03 Drilling/Alteration No Violations Noted Currently drilling 8 1/2" hole at a depth of 12,827' no issues at time of inspection. Site was clean but wet from rain. Also have vac trucks pumping out cellars and containment.

Bop tested 9/22/2018 (PASS).
