2712670 Primary Facility 2018-03-30 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Cannella well site on March 30, 2018. The site had a long access road which was solid and stable. The pad surface had a bit of standing water due to recent rains. The cut and fill slopes were well vegetated, to the point of having woody plant growth in abundance. The wells were properly labeled. The wells associated with this site are as follows:

115-20396 115-20397 115-20439 115-20498 115-20499 115-20500.

There were no violations noted.

2770876 Primary Facility 2018-08-29 Drilling/Alteration No Violations Noted - Arrived on location at 15:30. Onsite to check on (3) new spud notifications received for the Cannella 6H, 24HC & 25HC wells. Upon arrival Chesapeake Energy and Integrity Energy reps. were onsite finishing up setting temp., retrievable bridge plugs in the (3) existing wells on pad, the Cannella 1H, 2H & 5H. These wells had been previously producing and have been temp. plugged and disconnected from all production equipment in preparation for drilling new wells on pad.

- (3) new 20" conductors and 16" mouse holes have been installed. 20" conductors have been set to ~ 80'. No spudder rig was on location at time of inspection. - (8) Baker tanks and several lighting plants have been staged on location in preparation for future drilling. No liner or drilling mats have been set on location yet. - No obvious concerns or violations were noted at time of inspection. Left location at 16:00.
