1887710 Primary Facility 2010-04-01 Routine/Complete Inspection Violation(s) Noted The Department conducted an inspection in relation to discharge that is occurring on the back left corner of the site. The water has a slight dark tint to it and noticeable petroleum product sheen on top. The Department spoke with Rick Jones, company man on site. His initial belief is that the operator encountered a spring when building the location. This is what caused the discharge but to his knowledge, it had no contaminates present. The Department noted that a barrel of methanol was located 5 feet down slope from the discharge. The Department asked the operator to get the barrel out of the drainage swell. All of which point to a leak in the drilling pit.
1887833 Primary Facility 2010-04-08 Complaint Inspection Violation(s) Noted Complaint # 270271 was responded to by WQS Miller . Complainant' s water supply is within 1000 ft of gas well. Recently Antero had some production fluids released to the ground (drill cutting liquids, motor oil, and diesel fuel). Samples taken on 4/8/2010 show elevated parameters from pre-drill samples taken on 11/11/09. The Department has determined that the water supply has been impacted and that the operator is now required to replace and restore
1887902 Primary Facility 2010-04-08 Follow-up Inspection Outstanding Violations - No Viols Req'd The Department met with Jeff Woodcock from Civil Environmental Consultants and split sampled. Results are attached. Furthermore, the Department also sampled for 942 analysis on 4/7/10. Sample number 0499-017 was taken at the sediment trap. Preliminary test results from samples that were taken on 04/01/2010 show elevated conductivity, iron, manganese, and TDS. Rick Jones from Antero, stated that a diesel spill had happened underneath the rig floor. Sample number 0499-017 was taken at the sediment trap.
1887934 Primary Facility 2010-04-29 Follow-up Inspection Violation(s) & Recurring Violations
1888806 Primary Facility 2010-05-07 Routine/Complete Inspection Violation(s) Noted An NOV was issued for spilling motor oil. Citations included 78.54 and SWMA Section 301. Mike Ward from Antero called to report a release of motor oil from a generator he discovered during a routine operations inspection. Upon arrival WQS Miller observed motor oil seeping out of the slope on the West and North sides of the well pad and into the sediment trap below the site. A review of the tool pusher's logs found that generator #1 had used 89 gallons of oil from 4/21/10-5/3/2010. Antero was keeping the oil realease on site with absorbent pads and socks. Al Anderson from Ryan Environmental was present and Miller gave him permission to install a floating under drain system in front of the sediment trap which would separate the oil from the water. All of the pads and waste oil would be collecterd in drums and bthen be sent for disposal. The water that was sent into the sediment trap via the under drain would be pumped into thanker trucks and sent out for disposal. DEP requested that Antero retain all analysis and diposal slips for review.
1888839 Primary Facility 2010-05-13 Follow-up Inspection Outstanding Violations - No Viols Req'd The Department met with Jerry Alberts from Antero, and his contractors to discuss progress being made on the Hill Unit 2H site remediation. The initial seep located at the back left corner of the site now is lined and being pumped down periodically. The spilled cuttings have been cleaned up and put into a roll off dumpster to be taken to a landfill and be disposed of properly. SE Environmental is doing daily inpsections of the site and documenting every problem area that arises. The Department requested a copy of the daily logs. Futhermore, the operator has requested another meeting to discuss long term site remediation requirements. Seeps are still discharging discolored water and a recent 80 to 100 gallon motor oil spill is now also working out of the pad toward the sediment trap.
1888846 Primary Facility 2010-05-20 Follow-up Inspection Outstanding Violations - No Viols Req'd A follow-up inspection was conducted to check remediation status. The rig and all the equipment have been taken off the site. Trailer trucks were removing the last bit of pipe and flanges from the site. The pit/sump has been collected oil for a few days and the oil seeps from the slopes have stopped. Mike Ward from Antero expects to start removing contaminated soil from the site within the next couple of days.

    -The depression around well 2H was filled with water.
    -No evidence was found of spills or leaks on the site at the time of inspection.

•J M Sphar 4: The well and surrounding area were in good condition with no evidence of spills or leakage. Vegetation and berm around the well head and associated tank were also intact.
