Moved onto this well on 3/13/17 and resumed drilling at 53 ft. KB. The 18 5/8” casing was set at 246 ft. on 3/14/17 and cemented to surface with 40 bbls returned. The 13 3/8” casing was set at 342 ft. on 3/15/17 and grouted to surface above a basket. Coal was found from 286 ft. to 293 ft. A basket was set at 275 ft. Currently at 484 ft. and drilling ahead.

The 9 5/8” casing was set at 1478 ft. on 3/18/17. A Casing Annulus Packer was set from 999 ft. to 1009 ft. in the 9 5/8” casing string. The operator planned on setting casing at 1580 ft. The operator was not able to displace cement to surface. The estimated TOC is at 344 ft. The operator was only able to get grout tubing to 150 ft. KB to grout the casing to surface.

Reentered this well on 3/26/17 to run a SBT Bond Log after attempting to grout the 9 5/8” casing to surface. The log was completed and sent to operator’s office for evaluation. The well remains at 1640 ft. KB.

Operator ran second grout tube in the annular space between the 13 3/8” casing and the 9 5/8” casing to grout the 9 5/8” casing to surface. Operator was unable to get either of two (2) grout tubes below 150 ft. KB. TOC is at 344 ft. after first attempt to cement 9 5/8” casing. Operator ran several SBT Bond Logs after attempting to grout 9 5/8” casing to surface. The SBT Bond Log provided to the department shows inconsistent cement from 344 ft. to surface. Operator indicated poor grout job was due to water in well bore. After this attempt to correct defective cement job the operator resumed drilling. KOP was at 2000 ft. KB. Currently at 2781 ft. and drilling ahead.

2720115 Primary Facility 2018-04-19 Incident- Response to Accident or Event In Compliance with Policy On 4/19/18 an incident inspection of the Vivian Harbison Well Site was conducted in response to CTS ID: 333587. The following was observed:

>>On 4/13/18 EQT's Mike Cromley reported a ten gallon spill with cleanup beginning immediately as a vacc truck was present on site at the time of the spill.

>>Further communication with operator personnel indicated a spill of 10 gallons of produced water near the offloading point near the production tanks and that the spill cleanup was completed the same day.

>>Site inspection found no visible evidence of contamination, cleanup appears to have been fully completed in compliance with policy.
