2742991 Primary Facility 2018-05-09 Routine/Complete Inspection No Violations Noted On 5/9/18 a routine partial inspection of the Kevech pad was conducted in response to drilling notifications and reports of significant activity at the site. The following was observed:

>>Construction of an AST pad was being conducted at the time of inspection. Multiple sections of CFS located around the construction area were noted to be poorly installed leaving gaps underneath or between sections. No violation, immediate remediation is encouraged due to heavy storms predicted for the area.

>>Large amounts of accumulated clay dust on the access road between the area of the new construction and the existing pad may present a safety hazard when wet as well as increased risk of sedimentation in runoff.

>>At the time of inspection operator personnel reported initial cement work had just been completed prior to cellar installation.

2745756 Primary Facility 2018-06-26 Drilling/Alteration No Violations Noted Decker Drilling Rig #11 moved onto this well on 6/24/18 and resumed drilling at 53 ft. KB. The 18 5/8" casing was set at 306.6 ft. on 6/25/18 and cemented to surface with 53 bbls returned. The 13 3/8" casing was set at 418 ft. on 6/26/18. Operator was able to circulate gel to surface but not cement. TOC was at 53 ft. Casing was then grouted to surface. Coal was found from 351 ft. to 358 ft. A basket was set at 297 ft. Currently at 458 ft. and waiting on cement.
2748873 Primary Facility 2018-07-05 Drilling/Alteration No Violations Noted Decker Drilling Rig #11. The 9 5/8" casing was set at 2008 ft. on 6/29/18 and cemented to surface with 48 bbls returned. A Casing Annulus Packer was set in the 9 5/8" casing string from 1362 ft. to 1372 ft. KOP was at 2066 ft. TD was at 4984 ft. on 7/3/18.
2756527 Primary Facility 2018-05-24 Follow-up Inspection No Violations Noted On 5/24/18 a follow up inspection of the Kevech pad was conducted in relation to issues noted in a previous inspection. The following was observed:

>>Deficiencies in implementation of site BMPs had been addressed and satisfactorily remedied. No further concerns were found.

>>Construction of site accessory areas was ongoing at the time of inspection.

>>A section of redundant silt fence was noted to be constructed along an exposed fill slope above the access road, consultation with EQT's Jason Tyler indicated that the fence had been put in place to catch "clumpies". The fence did not change function or flow of water on site. No violation.

2757119 Primary Facility 2018-07-26 Drilling/Alteration No Violations Noted On 7/26/18 a routine complete inspection was conducted of the Kevech well site. The following was observed:

>>A large soil stockpile near the impoundment area of the site was lacking stabilization and appeared to be suffering minor erosion damage in the area used to access the pile. No controls directly down gradient from the access could allow runoff and material to enter the access road and leave the site. Remediation strongly encouraged.

>>Substantial sediment was found accumulated in the collection channel between the large soil stockpile and main access road. Any runoff would likely carry sediment into the pictured under road culvert and into the vegetated strip on the side of the road. Compromised CFS with excess sediment overtopping the control was found directly up gradient along the channel. It is likely the source of the the accumulated sediment. Remediation recommended.

>>Cleanup from drilling activities were ongoing at the time of inspection.

>>Black mud in containment where panels w

2757231 Primary Facility 2018-07-27 Follow-up Inspection No Violations Noted On 7/27/18 a follow up inspection of the Kevech well site was conducted in response to issues noted during a previous inspection. The following was observed:

>>The access point to the large soil stock pile was being stabilized at the time of inspection.

>>CFS between the stock pile and the access road had been cleaned and repaired with added surface matting added above it.

>>The production fluids remained in the containment unit but spilled fluids and contaminated pad materials had been removed.

2789777 Primary Facility 2018-10-16 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #292 moved onto this well on 10/3/18 and resumed drilling at 4999 ft. KB. KOP was at 8520 ft. EOB was at 9694 ft. MD (7577 ft. TVD). TD was at 17,191 ft. MD (7631.5 ft. TVD) on 10/15/18. Currently circulating prior to TOOH to run 5 1/2" casing.
2795328 Primary Facility 2018-10-26 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #292 got drill string stuck in well bore TOOH. Started packing off on the morning of 10/17/18. Had wireline check free point on the morning of 10/18/18. Began attempts to fish drill string on 10/19/18. Ran overshot tool and checked free point several times. Operator unable to remove entire drill string from well bore. Top of fish at 13,733 ft. Bit at 13,997 ft. There is 264 ft. of the drill string left in well bore (5 joints and BHA with MWD tool). Started open hole side track at 12:30 pm on 10/25/18 at 10,071. Currently at 10,173 ft. and TOOH 200 ft. before continuing side track.
2797420 Primary Facility 2018-11-01 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #292. Drilled to 12,152 ft. on 10/30/18. Short tripped and was unable to get back into sidetrack. Made attempts and drill string went into old well bore. Operator set plug from 9350 ft. to 8150 ft. on 11/1/18. Currently rigging up to run tubing and set kick plug.
2804340 Primary Facility 2018-11-16 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #292. Kick plug was set from 7550 ft. to 8150 ft. KOP was at 7468 ft. EOB was at 9668 ft. MD (7575 ft. TVD). TD was at 16,914 ft. MD (7627 ft. TVD) on 11/6/18. The 5 1/2" casing was set at 13,285 ft. due to BHA twisting off and being left in well bore. Estimated TOC at 4100 ft.
2853248 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Well appears to be well maintained. Well is producing.
