1930322 Primary Facility 2010-11-23 Follow-up Inspection Violation(s) Noted Onsite visit with DEP Geologist (Eric Rooney) and DEP WQS (Ryan Klemish). Followup inpsection of possible stray gas in cellar. Lower elbow on the 2" vent line of the 13 3/8" annulus had bubbles migrating out of joint. Small intermitent bubbles seen every 10 sec. Approx 6" of water in cellar at time of inspection. Asked DEP rep to use flir camera to detect any gas that may be present as I wasn't sure if bubbling from elbow was gas migration or plumbing problem. Also, Gauge on 13 3/8 showing 0 psi, however vavle closed during time of inspection and possible loose elbow. Talked with completions supervisor Nick Pottmeyer stating they would go out and tighten up fittings, open the vavle and monitor for any pressure buildup. Also asked Mr. Pottmeyer to replace the guage on the string with one with smaller graduations to detect any gas that may be present.

2348623 Primary Facility 2015-02-18 Follow-up Inspection No Violations Noted Chesapeake was in the process of breaking down secondary containment and washing rig mats. I recorded a conductivity reading of (3.59) milliSiemens/cm in water on containment on the western side of the pad near the cut slope. A vacuum truck operator was in the process of removing water on containment nearby area where the elevated conductivity was recorded. I pointed out to him the where the water with the elevated conductivity was. He claimed he would remove all of the water on containment. I recorded (3.62) milliSiemens/cm conductivity in water on the pad surface next to a heating unit and frac tank used to contain water to wash the rig mats. Mr. Wilkes informed that the water with the elevated conductivity readings off containment will be removed via and on-site vacuum truck.
2349427 Primary Facility 2015-02-26 Follow-up Inspection No Violations Noted Chesapeake was in the process of breaking down secondary containment, washing rig mats and removing snow and ice on containment. Mr. Krut informed me of a crack in the poly urea encompassing the perimeter of the 1H wellhead. The wellheads are contained with a cement apron. The cement below the poly urea on the 1H well is not compromised according to Mr. Krut. Due to the crack in the poly urea, Leidos Engineering performed soil conductivity screening on the pad surface around the perimeter of the cement apron on 2/25/15. Mr. Krut indicated that Leidos collected a total of (6) EC screenings/. The highest reading was (628) microSiemens/cm. Background soil conductivity readings, which were collected in the Northeast corner of the Pad, were (301) microSiemens/cm according to Mr. Krut. The Department did not observe staining/visibly impacted soils on the pad surface near the cement apron; N 41°38’22”; W-75°58’24”.
2349574 Primary Facility 2015-03-05 Administrative/File Review No Violations Noted This administrative inspection has been created to document Chesapeake’s discovery of soil conductivity screening readings above background levels on 2/26/15. On 3/2/15, Nick Krut, Environmental Representative of Chesapeake submitted an Electrical Conductivity Screening Summary Map for the McGraw Wyo Pad. Leidos Engineering performed soil conductivity screening at the site on 2/26/15. Due to soil conductivity readings elevated above background levels, the Department requested Chesapeake collect a sample in the area that exhibited the conductivity reading of (637) microSiemens/cm to verify the presence or absence of potentially impacted pad surface material. The Department requested the sample be analyzed for flowback/production fluid parameters. Chesapeake has agreed to collect a sample based on the Department’s request.
2349622 Primary Facility 2015-03-06 Follow-up Inspection No Violations Noted Chesapeake was in the process of attempting to return the McGraw 2H and McGraw 5H wells into production. Mr. Krut contacted me on 3-5-15 regarding the removal of all of the secondary containment used for hydraulic fracturing operations. The containment was completely removed from the site on 3-4-15. It has taken several weeks to remove the containment from the site due to the weather, according to Mr. Krut. The department did not observe any signs of contamination of the pad surface where the containment was staged. The majority of the pad surface was snow covered from the most recent snowfall event.
2359214 Primary Facility 2015-04-08 Administrative/File Review No Violations Noted This administrative inspection has been created to document Chesapeake’s submittal of a report to the Department on 4/2/15 containing a datatable and sample results of a sample collected by Chesapeake’s contractor, Leidos Engineering at the McGraw well Pad on 3/11/15 in response to the release of approximately (750) barrels of proppant laden fluid on containment on January 19, 2015, and elevated field electrical conductivity (EC) screenings, conducted by Leidos, on February 25, 2015 documented in the McGraw 3/5/15 inspection report. The Department has concluded that there are no technical deficiencies in the report containing the data table and sampling results and no further action is warranted at this time at the McGraw well pad in the area where the proppant laden fluid was released to containment and the field electrical conductivity screening readings were detected at levels slightly above background.

2692769 Primary Facility 2018-02-01 Routine/Partial Inspection No Violations Noted Onsite in follow-up to citizens report of activity occurring on pad. Upon arrival: Weather: Clear, cold, ~20 degs. (5) wells are present on this pad. The 1H, 2H, 3H & 5H were Producing. The 4H well head was being prepped for Stimulation. Weir Energy hands were onsite replacing the production tree of 4H with a 10K master valve. I spoke with CALLC's Completions WSS, Brian Heffron onsite. Mr. Heffron mentioned that the (4) producing wells would remain in production until the the Completions equipment was being mobilized to pad, which he thought would be several weeks. Access was limited to well area due to work being performed on pad. No obvious concerns or violations were noted at time of inspection.
