Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2207934 Primary Facility 2013-09-27 Routine/Partial Inspection No Violations Noted Pre-operational inspection.
2209749 Primary Facility 2013-10-03 Drilling/Alteration No Violations Noted United Drilling , will grout cond later today and drill mousehole tomorrow.
2215302 Primary Facility 2013-10-23 Drilling/Alteration No Violations Noted NOMAC 76, Comp Man - Josh Delso, @ 2000'
2219248 Primary Facility 2013-11-05 Drilling/Alteration No Violations Noted drilling at 12,472
2219470 Primary Facility 2013-11-01 Drilling/Alteration No Violations Noted A storage container in the backyard appeared to be right at the (2)’ freeboard requirement of Title 25, Env. Protection, Pa. code 78.56. It is a (200) barrel capacity container, most likely holding between (130-140) barrels. Mr. Delso mentioned the container is storing containment water. The fluid level will be lowered in the tank in the very near future and transported to the Sharer Facility for treatment and intent for beneficial reuse. As I was leaving the site at 14:50 I noticed some oil from the drill pipe cleaning procedures flowed off the plastic liner in place below the pipes into a puddle of water on the ground surface of the Pad.
2221120 Primary Facility 2013-11-08 Drilling/Alteration No Violations Noted NOMAC 76, Comp Man - Darren Naylor, @ 13670
2340505 Primary Facility 2015-01-23 Routine/Complete Inspection No Violations Noted Chesapeake was in the process of breaking down secondary containment as I arrived. While in the process of surveying the pad surface and secondary containment, I used an Oakton meter to record conductivity levels of ponded water on secondary containment in the eastern section of the pad at N-41°35’12”; W-76°05’21”. Conductivity readings were over range on the meter. The highest reading that can be recorded on the meter is (20) milliSiemens/cm. A skid steer was in the process of breaking down the containment in the area where the elevated conductivity was recorded in the water. The water was on the surface of a (60) mil liner made of plastic material. Rig Mats are in place below the liner.

2481632 Primary Facility 2016-04-28 Routine/Complete Inspection No Violations Noted On 4/26/16 Eric Hottenstein, Environmental Supervisor of Chesapeake Energy informed Matt Shope, PADEP O&G WQS, via e-mail the following. "We plan on discharging storm water from tank containments this Thursday 4/28/16 starting at 10:00 AM on the Tinna Well Pad in Wyoming County". Prior to the commencement of discharging the storm water in the production fluid tank containment structure, Darin Chaffee, Water Technician of CHK and I visually assessed the water in the containment for evidence of a visual sheen or discoloration, foam or scum buildup on the surface and a significant amount of sediment. We did not observe sheen, foam or scum buildup on the surface of the water, a significant amount of sediment or any unusual odors in the water. The water appeared to have some turbidity due to the fact that it may have been in the containment for quite some time and there were decomposing leaves and sediment settling to the bottom of the containment. At 10:40 AM, Mr. Chaffee commenced the process of discharging
