At time of inspection well does not appear equipped for production. Well tag present but does not include permit number (photo #2). No brine tank on site. Well cellar nearly full of fluid. A slight bubbling is occurring in the fluid in the well cellar. Due to this bubbling, this inspection is being referred to DEP OGI for follow up. It appears that earth disturbance activity has taken place on site. Temporary BMP's installed down-gradient of well pad. Site is not rocked or vegetated (photo #3). 1-10-19: Department contacted operator rep Jim Mills to discuss above issues. Mr. Mills states previous operator was preparing to plug the well when the well was acquired by Penn Energy. Mr. Mills states the well is currently scheduled to be plugged. Depa

2829092 Primary Facility 2019-01-15 Follow-up Inspection In Compliance with Policy FU ADMIN inspection to inspection record #2825938 where a violation was issued for failure to permanently stabilize site.

Operator rep Doug Mehan contacted Department by phone and left message. In this message Mr. Mehan states that well tag has been placed on the well head, fluid in well cellar has been removed, and that seeding and mulching the site will be completed on 1-17-19. As of this date Department has not received operator response to NOV issued on 1-9-19. Violation issued on 1-9-19 remains outstanding. A follow up inspection will be conducted at a later date.

2830915 Primary Facility 2019-01-22 Administrative/File Review In Compliance with Policy ADMIN inspection to inspection record #2825938 where a violation was issued for failure to permanently stabilize site.

Department received operator response to NOV (photo #1). Operator appears to be in compliance with policy at this time. Violation issued on 1-9-19 remains outstanding. A follow up inspection will be conducted at a later date.

2833607 Primary Facility 2019-01-28 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to inspection record #2825938 where a violation was issued for failure to permanently stabilize well site.

At time of inspection well tag present and labeled with correct information. Well head surrounded by security fence. Well cellar does not appear to contain fluid at this time. Site snow-covered but appears mulched at this time. No E and S issues observed at this time. According to Department records this well is in Regulatory Inactive status. Department contacted operator rep Doug Mehan by phone from site. Mr. Mehan states well is scheduled to be plugged in 2019. Operator appears to be in compliance at this time. Violation issued on 1-9-19 has been corrected.
