At time of inspection well does not appear equipped for production. Well tag present but does not include permit number (photo #2). No brine tank on site. Well cellar nearly full of fluid. A slight bubbling is occurring in the fluid in the well cellar. Due to this bubbling, this inspection is being referred to DEP OGI for follow up. It appears that earth disturbance activity has taken place on site. Temporary BMP's installed down-gradient of well pad. Site is not rocked or vegetated (photo #3). 1-10-19: Department contacted operator rep Jim Mills to discuss above issues.
