1947924 2011-01-12 No Violations Noted On January 12th, 2011 I conducted a routine inspection of the above site. Drilling was taking place at the time of the inspection. A site ID sign and E&S plan were posted at the site. The access road was in good condition. All E&S controls were in place and constructed according to the plan at site. Hay filter sock had been placed in behind the silt fence as an alternative to hay bales. A rip in the upper portion of the pit liner occurred the day earlier. The pit was drained and relined. No material escaped the drilling pit. The pit appears to be in good condition but its filling quickly. Water trucks were arriving on site at the time of inspection to begin hauling water away.

The new pit liner appears to be intact and holding well. No spills, leaks, or accelerated erosion were observed during the inspection.

The access road was in good condition. The pit was closed and the site grading has begun. No spills, leaks, or accelerated erosion were observed during the inspection. A well tag identifying the operator and permit number needs secured to the well.
1993784 2011-07-21 No Violations Noted THIS GAS WELL IS HOOKED UP AND PRODUCING. THE SITE IS NOT RESTORED. THE PIT IS GONE AND THE ACCESS IS STONED.
2107689 2012-10-16 No Violations Noted At the time of the inspection, the well site sign and label were on site. The well was drilled and equipped for production. The associated well apparatus located onsite appeared to be in sound working condition. There were no erosion or sedimentation issues. The area around the pad was restored and meets the 70% uniform vegetative coverage.
