1851523 2009-12-22 No Violations Noted At the time of inspection, the well was drilled but not set for production. Site was properly signed with E&S plan available for review. Access road was in stable condition. No E&S concerns with access road or well pad. Bmps were properly installed and functioning. Noted two pits onsite, recommended that operator monitor the pit freeboard as levels were near the 2ft of freeboard. I also recommend the operator utilized better measures to secure pit liners in order to prevent a potential pollution event. Inspector Paul Wyrwas was also onsite at time of inspection and indicated that on 12/23/2009 operator had plans to frac the well.

Titan wire Line Company present to run geophysical study. Hydro frac is scheduled for 12/23/09. Well head set at 8¿ to 4¿ reducer and shut off. E&S controls were implemented. Well sign and E&S plan located at the entrance of site. At the time of inspection, no E&S issues or IW issues.
1941535 2011-01-06 No Violations Noted Well head consisted of 8¿ pipe reduced to 4 ¿pipe reduced to 2¿ production line. No signs of leakage from well head. Production equipment consisted of heater, separator (200 psi) and 100 bbl brine tank (< 10 bbl fluid). At the time of inspection, no E&S and IW issues at the site. Totalized show 5366 MCF and 542 MCF.
