Dept rec'd area of earth disturbance is re-rocked to ensure stabilization is maintained.Mr. Small also stated new well tags will be installed immediately.

2826985 Primary Facility 2019-01-15 Routine/Complete Inspection No Violations Noted On 1/15/19 at 1127 hrs., I conducted a routine inspection of Chevron Appalachia, LLC's Buterbaugh 10H well (API 063-37021) in Green Twp., Indiana Co. Snow cover prevented proper evaluation of site stabilization. The well pad appears to be restored and permanently stabilized. Three production fluid tanks are located within adequate secondary containment. Three wells are drilled and equipped for production. A site ID sign and permit box are posted at the site entrance. It is unclear as to whether or not well tags are affixed to any of the well heads as the perimeter fence to the well pad is locked. The Department requests that the operator provide either a combination or key to one of the locks to facilitate access to the site. This should be sent to the Department inspector listed on this report. There are no violations noted for this inspection.
