2075700 2012-06-07 Outstanding Violations - No Viols Req'd A follow up inspection was conducted at the EQT Pad S Flowback Impoundment 2 site at approximately 1100 hours on June 7, 2012. The purpose of this visit was to evaluate the status of flowback removal from Impoundment 2 and to identify the extent of an elevated reading of specific conductance discovered earlier in the week.

Earlier in the week a seep entering the road ditch at 41°38’47”N, 77°14’55”W was discovered and exhibited an elevated reading of specific conductance, a sample was collected and submitted for laboratory analysis at that time. During this inspection, the flow measured of 5.41 mS. on the field conductivity meter and was followed up gradient. At 41°38’48”N, 77°15’00”W the flow at the road culvert above the road was measured and exhibited a reading of 8.59 mS. The upland slope that contributed flow to this culvert is the slope southeast of EQT’s Impoundment S. This location is approximately 1,700 feet from the Southeast corner of the Impoundment on a heading of 115°.

The discharge of spring @ culvert (41° 39' 2.48"N, 77° 15' 8.49"W) = 14,500 uS/cm The unnamed tributary to Rock Run (originating at the sand spring) at the road culvert (41° 39' 07"N, 77° 14' 10"W) (This is also referred to as C&A Station W). = 1,900 uS/cm. The clean UNT to the north of C&A Station W - 52 uS/cm UNT culvert; The two unnamed tributaries came together and went under the road through a culvert (41° 39' 7.2"N, 77° 15' 10.368" W) = 937 uS/cm The unnamed tributary was followed from the road culvert to the mouth where it emptied into Rock Run. The readings were1,040uS near the road and remained around 850uS - 900 uS all the way downstream at which point it emptied into Rock Run with a conductivity of 801 uS/cm Rock Run upstream of the UNT (41° 39' 06"N, 77° 14' 53"W) = 30 uS/cm Rock Run Downstream (41° 38' 43"N, 77° 14' 43"W). 222uS/cm

2076601 2012-06-06 Outstanding Violations - No Viols Req'd A follow up inspection was conducted at the EQT Pad S Flowback Impoundment 2 site from approximately 1045hours to 1500 hours on June 6, 2012. Representatives of the Department included Terra Tokarz (Water Quality Specialist), Jessica Ritenour (Professional Geologist ), Randy Farmeri (Professional Geologist Manager) and myself . Steve Kepler (Fisheries Biologist) and Heather Smiles (Fisheries Biologist) from the PA Fish and Boat Commission (PA F&BC ), were also present. Dave Allison and Mike Cromley from EQT were on-site.
2076602 2012-06-13 Outstanding Violations - No Viols Req'd A follow up inspection was conducted at the EQT Pad S Flowback Impoundment 2 site at approximately 1015 hours on June 13, 2012. The purpose of this visit was to evaluate the status of flowback removal from Impoundment 2, to identify the extent of an elevated reading of specific conductance discovered during previous inspections and to establish routine sampling points.

At the time of this inspection no flowback remained in the pit. Workers are seen in the attached photograph pressure washing the liner. Also noted were many holes identified by orange markings. A trench that was excavated to drain the seeps to the west of the impoundment was completed and the fluid was being pumped to a 500 barrel tank on site. C&A were preparing to dig additional sumps / trenches in the vicinity of 41° 38' 48" N, 77° 15' 0 W" to remove more liquid with elevated conductivity.
