Access road gated and locked. At the time of inspection well appears equipped for production. Well tag present and affixed in a permanent manner. Brine tank located on site and appears full. During inspection, Department observed two active leaking points. First active leak observed at the T-joint connector of the inflow flange located at the top of the brine tank. An approximate area measuring 4’x4’ directly below the leak appears to be impacted, as indicated by visible staining of the ground/vegetation. Second active leak observed at the outflow flange located at the base of the brine tank. An approximate area measuring 2’x2’ directly below the leaking outflow flange appears to be impacted, as indicated by visible dampness and pooling of fluids, with no evidence of sheen. Production chart located onsite is dated 8-20-15.
