0256526653 2015-08-12 Orphan No Violations On site with Keith Moore with Moore Well Services to inspect the leaking well, located under the Lowzinskis patio, for a future orphan vault/vent project. The well is 22 inches from the patio door at the rear of Lowzinskis residence and partially covered with cement. The gas scope showed 3% LEL at the exposed casing under the cement seal. A very faint occasional gas odor could be detected right at the casing. The vault/vent procedure was reviewed with the landowners, Mr. and Mrs. Lowzinski. The patio currently has cement pavers around the partially exposed well. Lowzinskis will decide in the near future the grade they will need the vault manhole to be so that it is level and accessible with this or a new patio that is planned to be built in the future.
1276882362 2015-08-18 Orphan No Violations Orphan Well Manager, Gene Chini, and myself met with the landowners, Mr. and Mrs. Lowzinski, to review the future vault/vent project proposed for the leaking well located 22 inches behind Lowzinskis residence under their patio. Lowzinskis filled in the excavated area around the well with river rock for safety reasons. This area had been excavated by Dominion Gas to determine the source of a high gas reading in the patio pavers. Dominion located an old gas well under the cement pavers. The casing was covered with a cement seal and part of the casing was exposed by the excavation. Lowzinskis are considering replacing the patio and will determine what grade will be necessary for the manhole cover on the vault. It was explained that the Orphan well program can not build a new patio but will repair any damage caused by the vault project and replace pavers in the vault area.
1296479707 2017-03-06 Orphan No Violations Todays inspection was conducted for the Technical Team to review the well site. The status of the well has not changed since the last inspection.
1440681900 2015-06-24 Orphan No Violations This house is used as a vacation home by the landowners, Mr. and Mrs. Lowzinski. They noticed an occassional gas odor around 06-11-2015 while sitting on the patio. Dominion Gas was called and determined the source of the gas odor was not from Dominions gas lines in the area or from Lowzinskis Dominion gas line for their house. Dominion had a true high gas reading in one spot on the patio and excavated in this area.. One side of what appears to be well casing is evident in the south area of excavation. Dominion did not have significant readings anywhere but at the well casing An approximate 36 inch diameter patch of cement is over the casing.The well casing is 22 inches east of the patio door for the family room. The house was built in 1960 and Mr. Lowzinskis mother bought in in 1988 with no knowledge of the well. A neighbor recently told Wesley Lowzinski that the original homeowner discovered the well when he cleared the lot to build the house. Mr. Lowzinski contacted the original owner, Lois Madonna, and a relative of Madonnas, who said the well was sealed when the attached garage was made into a family room in 1970. It is unknown how the well was sealed. The visible well casing maybe an 8 5/8 diameter casing.The gas scope reading today with the Divisions gas scope was 2% on the south side of the exposed casing. An occassional, very faint gas odor was evident just at the well casing. Complaint #5058
