0064167627 2017-03-14 Production Wells Violations Noted An anonymous complaint made its way to me from the Columbus Office, reporting a tank that had ran over and spilled oil and brine into the dike for this well. The complaint had pictures accompanying it. I arrived at the location and noticed part of the dike had been disturbed. I then walked up the side of the dike and could see the oil floating in the dike. I also could see where dirt was spread in the dike seemingly to cover up the contamination. The line from the knockout to the plastic water tank was laying on the upper side outside the dike, having been disconnected. All contaminated soil shall be removed and disposed of in an EPA approved landfill. Any soil outside the dike that is contaminated with brine, as to prevent the growth of vegetation, will need removed also. In rebuilding the dike a dike drain would be highly recommended to ensure the dike can be kept reasonably free of fresh water. No such measures were evident at the time of my inspection, perhaps it was covered in the attempt to cover the contaminated area inside the dike with uncontaminated soil. The site will then be seeded and mulched to allow vegetation to reestablish and prevent erosion. This event would also be in violation of the mandatory Incident Notification that was signed into law by Governor Kasich in August of last year.

0365448589 2017-04-14 Production Wells No Violations Follow-up on this compliance notice found the location to have been excavated and the flow lines re-plumbed. The 40 barrel plastic/poly tank was removed from the dike. All oily substances were removed and at this time the violation was resolved.

0895641336 2017-10-25 Production Wells Violations Noted The Washington County inspector forwarded me an email he received about a release of oil from 1/2 to 1 barrel amount. I traveled to the wells location and found the tank battery to be covered in peatsorb, an area of 10x15 was covered. There did not appear to be any standing oil or free oil. A test for chlorides using Quantabs revealed only 314 ppm chlorides. I then traveled to a location where I was able to obtain cell service and called Andrew from Am-Tek Oil. I explained that the peatsorb will need raked up and the location remediated. He said he would have it done before the end of the week.
1020397237 2018-03-16 Production Wells No Violations This well was written up for pollution and contamination at the tank battery. The necessary remediation has yet to be completed. I will email the landowner who accepted responsibility for the mess to see when he will get to cleaning the location.
1177364901 2018-02-12 Production Wells No Violations I emailed the land owner who the well owner claimed to have caused the contamination. This was his response. Good morning Matt,

I am partially responsible for that spill. One of Amteks guys had nicked the line with a shovel and buried it without informing anyone. I dont believe the owner was aware of the nick. I will take full responsibility for the cleanup. I Managed to contain the spill as soon as it happened. With the exception of the vegetation and the runoff from the side of the tank. 1518183446 2018-02-09 Production Wells No Violations The side of the tank is still covered in oil and the absorbent pad is filled with oil at the base of the tank. The vegetation is still covered in oil and has not been removed.
1533124846 2017-11-01 Production Wells No Violations None of the peatsorb has been removed. The only noticeable change is that a walking plank was added to the dike.
1947794095 2018-01-31 Production Wells Violations Noted Inspection of this well found a well equipped with a pumping unit, tubing, rods, and an electric motor. Identification was faded at the wellhead and the tank battery. Identification will be made to be legible. Inside the dike there was evidence of a release. There was still oily vegetation on and near the dike. The contaminated vegetation will need removed. I called Andrew Ulrich the owner of Am-Tek Oil Inc. and left a message asking for a return call about the Mullins well.
