0300837579 2013-06-27 Orphan No Violations After receiving verification from ODOT that they only have an easement and CSX is the landowner, I e-mailed Sara Pepper (ODOT) to see if they have an agreement no. which is required before we can complete the form to do construction & maintenance on their land, on which the leaking gas well is located. If ODOT has no number, we will need to send a certified letter from the contract holder giving CSX permission to provide us with a copy of the agreement. This also requires check or credit card payment. Once we get the number, we can fill out the form for work on the well and the form takes 30 days to process. Letter stating this process is included in packet.

0906470411 2013-07-12 Orphan No Violations I received call from Russ Mouney this morning saying the Buchtel well is getting worse. It is surging a lot more frequently, sometimes to a height of 6, and the gas odor is getting stronger. He feels that it is not a healthy situation for the people and pets in the area and he had hoped we would have plugged it by now. I called Altier Brothers and asked him to call Orphan Well Program Manager Gene Chini and to discuss the installation cost for an above ground vault and vent system work to temporarily mitigate the issue at this site. The installation of this equipment was approved and scheduled for Monday morning. DOGRM Inspector Josh Garland will inspect the well this afternoon and notify Mr. Mouney and other nearby residents of the upcoming equipment installation.

1327227004 2014-06-27 Orphan No Violations The Altier Bros. crew arrived this day at 7:45 AM with myself, Formation Cementing and the water truck arriving at 8 AM. The tools were run into the 2 diameter tubing and established that the top of the cement was at a depth of 245. Next the crew switched out cable lines and removed the large square of concrete that was on top of the well. A platform was then fabricated over the well for the slips to hold the tubing. The logging truck arrived at 10 AM and shot off the 2 diameter tubing at a depth of 227. The tubing was then pulled, cutting the sections apart instead of unscrewing due the condition of the tubing. Once the 2 diameter tubing was removed, 1 diameter tubing was run into the 8 casing, past the shot point, to a depth of 360 and the 2nd cement plug was set to a depth of 210. Then the 1 diameter tubing pulled to 150 and the 3rd stage of cement was set to surface. All of the 1 diameter tubing was removed and the well was topped off with cement. There was no immediate drop in the cement level. A total of 88 sacks cement were used with the cement testing 13.3 - 13.6#/gallon. The crew left location at 3 PM.
1369257244 2013-05-22 Orphan No Violations 911 Athens County Dispatch contacted me at 1230 hrs about the condition of this well. It was leaking gas and periodically spitting brine about three feet in the air. I talked to Fire Chief Dean Russell (740-591-0542), Nelsonville FD whose department initially responded to the call, about the conditions at the well site. After discussing the situation with Josh Garland I responded to the site to see if there was anyway to control the leak. The wellhead was gone. All that remained of the well was a 2-1/2 inch tubing rising approximately 3-inches out of a 2 long X 2 wide X 1 deep concrete pad. It was continually leaking gas slowly and about every 4 to 5 minute period it would spit brine 3 into the air. The fire department personnel had built a small dike around the pad and placed adsorbent pads and pigs around it. I used Quantabs to test the chlorides in the brine. The tab measured chlorides above 6508 ppm. After some discussion with Fire Chief Scott McManus (740-277-8669) York Township FD and Chief Russell, Chief McManus agreed to provide the resources to build a more substantial dike on the down slope side of the pad to prevent any fugitive emissions migrating to the creek below this site which ultimately drains into the Hocking River. They also erected two signs, one No Smoking and the other No Trespassing, facing the apartments across the parking lot from the well site to try and keep people away from the wellhead. The fire department personnel, for safety purposes, also attempted to ignite the brine with a flare to make sure there was no oil or distillates. There was no reaction. Other equipment remaining for the well was a small knockout tank and line, neither of which were connected to anything. Gene Chini called me about this situation which he learned from Cindy Van Dyke.. He plans to visit the site tomorrow.
1369837721 2013-05-23 Orphan No Violations I spent morning at well with Josh Garland and Gene Chini, reviewing the situation and gathering information for the I/O submittal. Morgan Button was called out 5/22/13 on an emergency call of gas and brine leaking from a well in Right of Way of US Route 78 in town of Buchtel, OH. Morgan addressed the emergency situation and we returned this day to complete. The well was releasing fair gas and occasional bursts of brine thru 2 1/4 OD tubing that was rotten off about 6 above the square concrete base cemented in the ground. Within 10 was a small drip or 1 bbl separator with 1/2 fittings, not connected, and 1 and 2 valves coming out of the ground near the separator. I suspect the well is equipped with 2 tubing on a packer, which will have to be removed when plugged. The well lays barely within the highway ROW with Buchtel Apts, owned by Athens Metropolitan Housing Authority the owner, across the line. The well is 15 from their parking lot and 43 from the edge of the highway. There is a hole at the base of the separator and all other lines that supplied gas are not connected. There is an electric line over the well and trees nearby that will have to be removed. We talked with John Sullivan, the Mayor, and several other residents of the apartments and the town. The well had started unloading on 5/22/13 and last time it did this was 2005, when the well was given an I/O case number but the referral was never finished. Booms were laid down to contain/absorb the brine and pictures taken. SEM is ________. Visited the courthouse in the afternoon to begin title/lease search.

1373651323 2013-07-12 Orphan No Violations The residents nearby called to report in increase in the frequency of the release of gas and brine from the well. Upon inspection the well was noted to be releasing gas but not brine. A gas meter was used and LEL levels at the well were fluctuating between zero and two. During my inspection a resident informed me he had been feeling nauseous and dizzy since Saturday, July 6th. Since that time the well has been ejecting gas and brine at a much higher frequency, every 2-5 minutes roughly. Pictures taken.
1373894209 2013-07-15 Orphan No Violations Arrived this morning to inspect the installation of an above ground vault and vent system on this well. Altier Brothers installed a steel 50 barrel tank adjacent to the well, plumbing the well into the tank and then installed a 10 vent pipe on the tank. The Altier crew spent 1.5 hours on the installation of the equipment and plumbing it to the well. Unit was checked prior to my leaving the location and was functioning properly. Pictures taken.

1377803986 2013-08-29 Orphan No Violations I spent 1.25 hrs this morning with Ben Altier, running a 1/2 stem into well to check depth. We found we could only get down to 660, with the Berea being 900+. It seems to set down on something solid and was gassing considerably when we went in the hole. The mast of rig was only 1 away from electric line and electric may need to be shut off when plugging the well. I believe the well was drilled to Berea and will need cleaned out before plugging. Duration includes travel.

1404304670 2014-06-26 Orphan No Violations The Altiers crew arrived today at 9 AM with 1 diameter tubing. The well was swabbed and found to be full of fluid. Four swabs dried up the hole and there was very little gas present. The crew switched cable lines and at 11 AM started running the 1 (l.3 OD) tubing into the 2 diameter tubing with a tapered joint on bottom. This tubing was run to a depth of 595. Formation Cementing arrived at noon and set the bottom plug with 12 sacks of cement. Because of the small diameter tubing used, the cement weight was 13.3 - 13.6#/gallon when tested. The cementers left at 2 PM. At 3 PM the rig crew attempted to tag the top of the cement, but could not locate it. All left location at 3:30 PM. No water truck this day.
1404749214 2014-07-02 Orphan No Violations My inspection this day was to verify that the cement had remained in place at top of the well bore and witness the placing of the steel well identification (ID) plate on top of the cement. DOGRM Inspector Garland and Ben from Altier Bros. were also present. After placing the ID plate, the hole was backfilled and rough graded by hand. The 50 barrel steel tank and associated piping used to vent the well was removed from location. The grass where the cement crew had to wash up in the yard appeared distressed. I will wait a week or so and see if it comes back before starting reclamation.

1541345343 2014-06-24 Orphan No Violations I arrived on location at 8 AM. The rig crew swabbed the 2 diameter tubing and found fluid at @ 300. The second swab dried up the well. The crew dug around and below the square of concrete to attempt to find surface casing. They found an additional 6 of concrete below grade and exposed 8.625 diameter surface casing below the concrete. The crew then ran the swab tool again at 9 AM and encountered fluid at a depth of 450. The swab line stopped at 600. Made plans to cement bottom plug tomorrow. Left location at 10 AM. Spent computer time on this well.
1550945006 2014-06-25 Orphan No Violations I arrived at 8 AM (crew was there at 7:30) and crew went into the 2 diameter tubing with an impression block. The impression block showed only that there was sand in the bottom of the hole at @600. Appalachian Logging also arrived at 8 AM and ran a gamma ray, collar locator and temperature log. They found the bottom of hole to be at 604 with a packer set at 527-8. They found no casing below the packer. From a depth of 572 to TD it appeared that the hole was shot. The temperature log showed an anomaly at a depth of 237. The hole was then loaded with fresh water and a bond log run which showed no bond to the 2 tubing from the packer to surface. We were not able to determine from the log how much surface casing was set in this well. Formation Cementing and operators water truck arrived at noon. We tried to bull-head a 12 sack plug of cement but when the cementers started pumping, water from the hole circulated before they could mix cement. Had only a pumping charge this day and crew went home with plans to run 1 tubing in the morning to cement thru.
