Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0005604i52 2002-01-29 Production Wells No Violations
0406612748 2017-07-20 Production Wells No Violations This well is equipped with 4.5-inch diameter production casing. It also has tubing and rods installed in the well. Attached to the rods was a pumping unit. The pumping unit had a motor on it that appeared to be powered by means of electric. There was an electric pole with a box beside the pumping unit with an electric line ran to the motor. The motor had belts that were attached from it to the pumping unit. The production area contained two 100 barrel production fluid storage tanks, a vertical mechanical separator, and a gravity separation unit on top of one of the 100 barrel tanks. Production records within the RBDMS system are current up to 2016. I spoke with the landowner concerning the well and she told me that domestic gas was currently being utlized from the well.
0791723982 2018-06-19 Production Wells No Violations On the day of my inspection I witnessed Production equipment included 2- 100 barrel tanks, and a vertical separator contained within an earthen dike. The well was equipped with 4.5-inch diameter production casing, and 1.5 inch diameter tubing with rods, and a Churchill Pumping unit with an electric motor. The well head was equipped with a domestic gas line on the opposite side of the production lines. There was a back pressure regulator or controlling device allowing gas to vent into the atmosphere. There was also a small area of oil contamination on the ground near the well head approximately 6 square feet. There were no identification tags on the well or production equipment. I contacted Nathan Levengood, and Bruce Levengood from Sound Energy and informed them of the problems and he said that he would remediate the problems in the next couple of days. I plan to check on the well later in the week to see if the violations have been taken care of.
