0295611539 2016-03-30 Production Wells Violations Noted On the day of my inspection I was following up on Chiefs Order 2014-138. The status of the well appears to be the same. The well was equipped with a pumping unit, tubing, and rods. The well had a gas leak coming from a pressure monitoring valve on the well. The tank battery consisted of a 100 barrel production fluid holding tank, a brine storage tank, and a mechanical separator. The valves at the tank battery were in the closed position. Production records have not been submitted since 2004 and the well is unable to produce in its current state. Photos were taken on the date of this inspection. The well owner/operator shall submit a written plugging schedule to Inspector Andrew Corder at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per OAC 1501:9-11-OS(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

0704400355 2018-10-18 Production Wells No Violations Orders #2017-356 & 2014-138 and violation # 295611539 follow up inspection found the well equipped with 4.5 diameter production casing and 1.900 diameter production tubing and the well set up to be produced by the means of a pumping unit driven by an electrical motor however the electrical meter has been pulled and the electrical service to the well removed. Attached to the casing and tubing well heads is 2 diameter flow lines with both check and shut off valves in place with both shut off valves in the open position. Both the tubing and casing well head flow lines tie into one 2 sales line at the well head. At the production facility is a mechanical separator with the valves in the open position, there is a gas sales meter present with no current chart in the meter. A single 100 barrel steel production tank is present with 74 fluid measured in the tank, also present is a 800 gallon plastic tank with 100 gallon visible. The flow line between the separator and 100 barrel tank is leaking inside the dike. Identification present still reflects the registered owner who has had no bond since the Division forfeited it on 06/27/2005. No commercial production was reported since 2004. No domestic use hook up was found at the well head or tank battery.

1268507948 2019-01-29 Production Wells No Violations Orders #2017-356 & 2014-138 and violation # 295611539 follow up inspection. On this day I met with one of the 6 landowners identified as a Mr. . Audie Bednarczyk who indicated the family was receiving domestic gas from the well to heat the original family farm house on the property. We visited the tank battery site which consisted of a gas sales meter and mechanical separator along with a single 100 barrel steel tank, also present is a plastic brine tank. The well has a single 2 steel production line leading from the casing head to the separator, the valve on the casing head is open and the valves at the separator are open going to the meter. The meter has no current sales chart present. Mr. Bednarczyk indicated he has never identified where the house gas domestic hook up is located only the connection point of the house. The domestic hook up was not present at the tank battery or well head. Mr. Bednarczyk indicated his brother in law a Steve Kiss managed the paperwork for the family and could be reached at (937) 879-1465. I also discussed on this day with Mr. Bednarczyk the ownership of the well and if the family had interest in registering the well. No identification is present at the well head or tank battery and the well head is slightly leaking gas from a pressure gauge screwed into a 1/4 port on the 2 casing head production side outlet port.

The tank battery contained one 110 barrel oil storage tank, a 25 barrel brine storage tank, and a mechanical separator. This well is lacking proper identification. The well is capable of production, however production has not been submitted since 2004. Photos taken to document findings.
