The pad is in satisfactory condition with proper erosion and sedimentation controls in place. Company shall continue to monitor the location to ensure the pad remains in satisfactory condition. Preliminary restoration passed.
1091708393 2018-04-09 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 1900 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1300 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

Evolution water trucks were on site and were withdrawing fluid from the production area.
1588021984 2016-02-10 Production Wells No Violations On this day well was equipped to be capable of production. Trucks from Halliburton and Weatherford were still staged on the pad. The tank battery area contained two 400 barrel production fluid holding tanks. The site appeared to be in satisfactory condition.
1857289084 2018-08-15 Production Wells No Violations On the day of my inspection I witnessed that the well was currently in normal production operations with Annular pressure monitoring in place on both the 9.625-inch diameter casing gauge read 1570 pounds per square inch, the 13.375-inch diameter casing gauge was not present, I contacted EQT superintendent John Allender to inform him they needed to be monitoring all annular pressures. Production equipment included an individual Gas Processing unit with 2 additional sand traps, and 4- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There was also an additional 500-barrel portable fluid storage tank on location that was not hooked up to any other equipment and did not have secondary containment around it. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
