0098333756 2018-08-07 Production Wells No Violations Status check inspection found the well pad surface and lease road has a gravel surface and is in good repair. There are 3 wells present on the pad all in production. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was 100 pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was zero psi. The 5.5 diameter production casing internal working pressure was 800 psi and the production flow line pressure was 20 psi. The well is equipped with production tubing and artificial lift technology. There is a single gas processing unit on location. At the right rear corner of the well pad is the production fluid facility consisting of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a Mark West midstream station at the left front corner entry point of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad.
0112046069 2017-12-18 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains four 400-

1424194843 2015-02-17 Casing String Witnessed No Violations On this day the 18.625 casing was ran and cemented into a 22 open hole at a depth of 356. There was 501 sks of class A cement ran and cement circulated to surface. See cement ticket for details. No violations noted.
1541011055 2015-09-11 Preliminary Restoration No Violations On the day of my inspection the erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed. Toe prep for these wells expected to commence on 09/18/201.
