1381214673 2014-11-12 Casing String Witnessed No Violations On the date of inspection I am on site to witness the surface casing string (18.625 inch) being topped off. The surface cement job that had occurred the previous night failed to bring cement to surface. I was informed by American Energy Utica (AEU) consultant Jaime Lopez that they had run a Temerature Log to determine that the top of cement was at approximately 300 feet in depth. Baker Hughes has run feet of 1 inch tubing between the 24 inch Mine String and the 18.625 inch Surface Casing String. Baker Hughes pumped 98bbl (456sk) of Class A cement, 15.6ppg, 1.2 yield, 2% CaCl. There were no cement returns to surface. The job should have taken approximately 46bbl of cement to see returns at surface. AEU will order more cement and pump until they bring cement to surface.
1943642093 2014-08-28 Drill / Deepen / Reopen No Violations On the day of my inpsection Express Energy drilled and cemented the 26 conductor casing. See cement ticket for details. No violations noted.
