1096361430 2018-11-05 Production Wells No Violations Status check inspection to update the well status. There are (6) wells on the pad-3 that are producing and 3 that have been drilled with production casing set. The three that were last drilled with production casing set are the # 1-# 3 & #5 wells. All 3 are in square type cellars and the well heads are not visible above ground level elevation. The said well along with the other 2 producing wells do have 2 diameter riser pipes present with electronic transducers attached for annular pressure monitoring. The riser pipes are attached to the A&B well head sections. The 3 producing wells are under steel cages. There is a natural gas compressor on the rear left hand corner (South West) side of the well pad. There is (2) gas processing units and 2 line heaters present on the well pad. The production facility consists of (5) 400 barrel tanks and one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area. A 3rd party midstream facility is being installed at the left front corner (South) corner of the well pad, the perimeter berm has been removed. There also is 35 water trucks parked loaded and idling on the well pad, the well pad is being used for a staging area for water trucks.

1409961169 2014-09-05 Casing String Witnessed No Violations Purpose of this inspection was to witness the cementing of the surface casing back to surface. Falcon 23 drilled a 24x475 open hole. 20x460 (94#/ft) casing set with basket at 249. Casing was centralized with 6 centralizers. Previous casing set was a 26 conductor to 100. Baker-Hughes is onsite with 516 sks of class A cement (15.6 ppg; 1.21 yield). Cement additives are 3% CaCl and .25#/sk celloflake. 30% excess ran in the open hole. 156 bbl of fresh water used for diplacement. 15 bbl of cement returned to surface. Floats held.

Falcon 23 drilled a 17.5x2137 open hole and set 13.625x2107 (54.5 #/ft) casing. Previous casing set is 20 to 460. Baker-hughes is onsite with 1712 sks of class A (15.6 ppg; 1.20 yield) to cement the casing back to surface. Cement additives are 1% CaCl and .25#/sk celloflake. 321 bbl of fresh water used for displacement. Good returns to surface.
