The well is piped to a gas processing unit and then to a production area that contains five 400-barrel production fluid holding tanks. 1224911964 2018-09-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 630 pounds per square inch, the 13.375-inch diameter surface casing gauge read 205 pounds per square inch. Production equipment included a sand trap, Line Heater, Gas Processing Unit. There were also 5- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS.

1413830721 2014-10-20 Casing String Witnessed No Violations Upon arrival onsite, Alpha Hunter Rig had one joint of 24 casing to run. Rig drilled a 28 open hole to 218 and set 218 of 24 casing. Previous casing set was a 30 conductor to 100. Operator claims no mine was encountered and circulation while drilling was not lost. Issues running last joint. Several failed attempts at washing it down. Signs of coal were encountered between 154-168. 100% excess cement ran on bottom balance job. Cement baskets set at 139 and 144. Allied oil and gas services onsite to cement casing back to surface with 363 sks of Class A (15.6ppg; 1.18 yield). Cement additives are as follows: 2% CaCl and .25#/sk celloflake. Pump truck failure resulted in delayed job. Left pad, Inspector Corder witnessed cement job
1414010198 2014-10-22 Casing String Witnessed No Violations Alpha Hunter Rig drilled a 440x22 open hole and set 440x20 casing. Otex was onsite to cement the surface casing back to surface with 450 sks of class A (15.6ppg; 1.2 yield) cement. Cement additives were 2% CaCl and .25#/sk celloflake. Casing was centralized and no baskets were ran. Previous casing set was a 24 mine string. 123 bbls of freshwater used to displace cement and 11 bbls of cement were returned to surface.

2075122867 2019-02-07 Preliminary Restoration No Violations Preliminary Restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad surface and lease road is stone construction and in good repair. The lease road is gated and the well pad has a perimeter earthen berm with a 8 chain link fence on the berm. Signage is posted at each of the 8 wells on the pad. All 8 wells have artificial lift production equipment installed on the production tree. Annular pressure monitoring is in place on the well head A&B sections. The production facility consists of 5 steel tanks capable of holding 500 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. 8 gas processing units are present on the well pad in 2 different locations.
