I performed an on-site evaluation to confirm the proper vegetation was established and erosion and sedimentation controls were maintained. This site has passed the preliminary restoration inspection.
0747458147 2018-07-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with surface, intermediate and 5.5 inch diameter production casing with 2.875 inch diameter tubing, the well appeared to be open flowing with no artificial lift technology in place. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 895 pounds per square inch, the 13.375 inch diameter surface casing gauge read 465 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included a dehydrator, 2 phase separators, contact tower, and 3- 400-barrel purge tanks shared with 34013207790000, 34013207780000, 34013210770000, 34013210780000, 34013210790000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.

1946279048 2014-10-22 Casing String Witnessed No Violations Patterson 349 drilled a 24x420 open hole and ran 20x410 casing at 94#/ft. Casing was centralized and a cement basket was placed at 211. Previous casing set was 30x100. Baker Hughes cemented the casing back to surface using 799 sacks of class A cement (15.6 ppg; 1.21 yield). Cement additives were 3% CaCl and .25#/sk celloflake. 133 bbl freshwater displacement and 12 bbl of good cement returned to surface.
