0824221991 2014-12-22 Casing String Witnessed No Violations On this day the 18.625 casing was ran and cemented in a 24 open hole to a depth of 357. Stingray was on site to cement the casing. See cement ticket for details. No violations noted.
0859903327 2018-03-16 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 450 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 600 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
0989635759 2015-01-07 Administrative Inspection No Violations On this day the 9.625 casing was ran and cemented in a 12.25 open hole to a depth of 7857. Stingray was on site to cement the casing. See cement ticket for details. No violations noted.
1169859110 2014-12-28 Casing String Witnessed No Violations On this day the 13.375 casing was ran and cemented in a 17.5 open hole to a depth of 2093. Stingray was on site to cement the casing. See cement ticket for details. No violations noted.
1243304988 2015-09-21 Preliminary Restoration No Violations On the day of my inspection completion operations were finalized and the rig down and clean process was underway. The erosion and sedimentation controls were of satisfactory condition and proper vegetation had been established. The preliminary restoration for this well has been completed.
1417911249 2014-12-06 Casing String Witnessed No Violations Inspector on site for a 7:00 pm call out to witness the conductor cement job with the following observed. The said well was spudded on 12/06/2014 by Rocky Mountain Drillings top hole rig with a 110 x 32 conductor borehole being drilled with fresh water encountered @ 64. Stingray Cement services were on location to cement the 26x 110 of casing using 254 sacks of class A-3% calcium .25 pps flake cement with 30% open hole cement excess used and circulation established prior to cementing with good cement returns to surface.
1420642561 2015-01-07 Casing String Witnessed No Violations On this day the 9.625 casing string was ran and cemented. They are running a two stage job on this casing string. No violations were noted.

1647984811 2018-10-23 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. At the time of my inspection annular pressure monitoring was in place and the 9.625-inch diameter casing was at 20 pounds per square inch, the 13.375-inch diameter casing pressure was 350 pounds per square inch. Production equipment included a individual Sand Trap, and Gas Processing Unit. As well as 4 400-barrel steel production fluid tanks, and a 50-barrel production fluid tank that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
2072072459 2015-01-23 Drill / Deepen / Reopen No Violations Current operations are circulating the hole bottoms up to clean the bore.
